右美托咪定联合臂丛神经阻滞麻醉在上肢骨折患者术中的麻醉效果研究

摘要
目的:探讨右美托咪定联合臂丛神经阻滞麻醉在上肢骨折患者术中的麻醉效果.方法:选取该院 2021 年 12月—2023年 1月收治的 98例上肢骨折患者为研究对象,按随机数字表法将其分为对照组与观察组,各 49例.对照组采用臂丛神经阻滞麻醉,观察组采用右美托咪定联合臂丛神经阻滞麻醉.对比 2组血流动力学指标、应激反应、麻醉效果、不良反应发生情况.结果:T1～4 时间点,观察组心率、血氧饱和度及平均动脉压指标均低于对照组,组间差异有统计学意义(P<0.05).T4 时间点,观察组皮质醇、去甲肾上腺素、促肾上腺皮质激素水平均低于对照组,组间差异有统计学意义(P<0.05).观察组感觉阻滞维持时间、运动阻滞维持时间均长于对照组,组间差异有统计学意义(P<0.05).2组不良反应发生情况对比,差异无统计学意义(P>0.05).结论:右美托咪定联合臂丛神经阻滞麻醉应用于上肢骨折患者手术治疗过程中,可取得更为理想的麻醉效果,积极减少对血流动力学的影响,有效延长阻滞时间,且安全性较高,具有应用价值.
Abstract
Objective:To investigate the anesthetic effect of dexmedetomidine combined with brachial plexus block anesthesia in patients with upper limb fracture.Methods:98 patients with upper limb fracture admitted to the hospital from December 2021 to January 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 49 cases in each group.The control group received brachial plexus block anesthesia,and the observation group received dexmedetomidine combined with brachial plexus block anesthesia.The hemodynamic indexes,stress response,anesthetic effect and adverse reactions were compared between the two groups.Results:T1～4,the heart rate,blood oxygen saturation and mean arterial pressure indexes of the observation group were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).At T4 time point,the levels of cortisol,norepinephrine and adrenocorticotropin in observation group were lower than those in control group,and the differences between groups were statistically significant(P<0.05).The maintenance time of sensory block and motor block in the observation group was longer than that in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the occurrence of adverse reactions between the two groups(P>0.05).Conclusion:Dexmedetomidine combined with brachial plexus block anesthesia in the surgical treatment of patients with upper limb fractures can achieve a more ideal anesthetic effect,actively reduce the influence on hemodynamics,effectively prolong the block time,and have high safety and application value.
