邓运明教授治疗骨折延迟愈合病例数据挖掘及学术思想探析

摘要
目的:运用数据挖掘方式探讨全国老中医学术思想继承工作指导老师邓运明教授治疗骨折延迟愈合的用药规律及学术思想.方法:导出 2015 年 1月-2022年 9 月邓运明教授所诊治的骨折延迟愈合病例信息,按照纳入和排除标准进行筛选,构建"邓运明-骨折延迟愈合数据库",利用Excel、R语言等软件工具进行数据挖掘,以表格及图片形式进行展示.结果:共纳入69 例病例,得到方剂 273 条,涉及舌象 27 条,脉象 13 条,证型 12 种,药物 138 味,四气以温、平为主,五味以甘、苦、辛为主,归经以肝、肾、脾、心为主,强关联药物组合 40 个,核心药物共现网络 29 个节点、332 条边,可能的新方 4 条.结论:数据挖掘结果体现了邓教授"首重气血,通达为先,滋补肝肾,续筋接骨"的骨折延迟愈合治疗原则,为未来诊治和研究骨折延迟愈合提供了思路,为进一步深入学习邓教授学术思想提供参考.
Abstract
Objective:To explore the medication patterns and academic ideologies of Professor Deng Yunming,a national mentor in the inheritance of traditional Chinese medicine academic thoughts,in the treatment of delayed healing in fractures using data mining methods.Methods:Data on cases of delayed healing in fractures treated by Professor Deng Yunming from January 2015 to September 2022 were exported.Cases were selected based on inclusion and exclusion criteria,and a"Deng Yunming-Fracture Delayed Healing Database"was constructed.Data mining was conducted using tools such as Excel and R language,and the results were presented in tables and charts.Results:69 cases were included,resulting in 273 prescriptions,involving 27 tongue manifestations,13 pulse manifestations,12 syndrome types,and 138 herbs,the main Qi of four properties were Warm and Neutral,while the primary tastes of five tastes were Sweet,Bitter,and Pungent,the main meridians involved were Liver,Kidney,Spleen,and Heart,40 highly correlated combinations of herbs were identified,forming a core co-occurrence network with 29 nodes and 332 edges,4 potential new prescriptions were suggested.Conclusion:Data mining results reflect Professor Deng's treatment principles for delayed healing in fractures,emphasizing the importance of Qi and Blood circulation,nourishing the Liver and Kidney,and connecting tendons and bones,this provides insight for future clinical treatment and research on delayed healing in fractures and serves as a reference for further study of Professor Deng's academic ideology.
