椎间植骨融合内固定术在退行性脊柱侧弯患者治疗中的效果探究

摘要
目的:探究椎间植骨融合内固定术在退行性脊柱侧弯患者治疗中的效果.方法:回顾性分析 2019 年1月-2020年2月于该院就诊的64例退行性脊柱侧弯患者资料,依据不同的手术方式将其分为对照组(n=32)和观察组(n=32).对照组采用传统椎管减压术治疗,观察组采用椎间植骨融合内固定术治疗.对比 2组的治疗总有效率、治疗前后的Cobb角与脊柱功能、炎性因子水平、不同时间的疼痛视觉模拟(VAS)评分及治疗后的并发症总发生率.结果:观察组治疗总有效率为 96.88%,高于对照组的78.13%,差异有统计学意义(P<0.05).治疗 2 个月后,观察组的Cobb角小于对照组,日本骨科协会评分高于对照组,组间差异有统计学意义(P<0.05).治疗 7 天后,观察组白细胞介素-6、白细胞介素-1β和肿瘤坏死因子-α水平均低于对照组,组间差异有统计学意义(P<0.05).治疗后 6、24、72 小时,观察组VAS评分均低于对照组,组间差异有统计学意义(P<0.05).观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05).结论:退行性脊柱侧弯患者接受椎间植骨融合内固定术治疗的效果较显著,不仅能明显改善脊柱侧弯症状,促进脊柱功能恢复,还能显著降低其炎性因子水平,减轻其术后疼痛程度,减少并发症的发生.
Abstract
Objective:To investigate the effect of transforaminal lumbar interbody fusion fixation in the treatment of patients with degenerative scoliosis.Methods:The data of 64 patients with degenerative scoliosis treated in our hospital from January 2019 to February 2020 were retrospectively analyzed,and they were divided into a control group(n=32)and an observation group(n=32)according to different surgical methods.The control group was treated with traditional decompression of spinal canal,and the observation group was treated with transforaminal lumbar interbody fusion fixation.The total effective rate,Cobb Angle and spinal function before and after treatment,inflammatory factor levels,the pain of Visual Analogue Score(VAS)at different times and the total incidence of complications after treatment were compared between the two groups.Results:The total effective rate of the observation group was 96.88%,higher than 78.13%of the control group,the difference was statistically significant(P<0.05).After 2 months of treatment,the Cobb Angle of the observation group was lower than that of the control group,and the Japanese Orthopaedic Association Score was higher than that of the controlgroup,with statistical significance between the two groups(P<0.05).After 7 days of treatment,the levels of interleukin-6,interleukin-1β and tumor necrosis factor-α in observation group were lower than those in control group,with statistical significance between the two groups(P<0.05).At 6,24 and 72 h after treatment,VAS scores in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The total incidence of complications in the observation groups was lower than that in the control group.and the difference was statistically significant(P<0.05).Conclusion:Patients with transforaminal lumbar interbody fusion fixation in the treatment,which can not only significantly improve scoliosis symptoms and promote the recovery of spinal function,but also significantly reduce the level of inflammatory factors,alleviate postoperative pain and reduce the occurrence of complications.
