推拿联合手法按摩对双踝骨折患者踝关节活动度及功能的影响
Effect of Massage Combined with Manual Massage on Ankle Joint Motion and Function in Patients with Double Ankle Fracture

摘要
目的:探讨推拿联合手法按摩对双踝骨折患者踝关节活动度及功能的影响.方法:选取2020年1月-2022年10月我院收治的 66 例开放式手术治疗的双踝骨折患者为研究对象,按入院先后顺序以数字编号信封法随机将其分为对照组与观察组,对照组(n=33)采用术后常规康复疗法,观察组(n=33)在此基础上采用推拿按摩疗法,比较 2 组临床疗效、平衡功能、踝关节功能、踝关节疼痛程度、活动度和生活质量.结果:观察组优良率为 81.82%,高于对照组的 57.58%,差异有统计学意义(P<0.05).治疗后,2 组Baird-Jackson踝关节评分、AOFAS踝-后足评分均大于治疗前,且观察组均大于对照组,差异有统计学意义(P<0.05).治疗后,2 组疼痛视觉模拟评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05).治疗后,2组背伸、趾屈、内翻、外翻角度均大于治疗前,且观察组均大于对照组,差异有统计学意义(P<0.05).治疗后,观察组Berg平衡量表评分高于对照组,差异有统计学意义(P<0.05).治疗后,观察组生理职能、躯体疼痛、整体健康、情感功能评分均高于对照组,组间差异有统计学意义(P<0.05).结论:推拿联合手法按摩能有效提高双踝骨折患者的术后康复效果,可进一步改善足踝功能和踝关节活动度,有效提升患者平衡能力和生活质量,值得在临床推广.
Abstract
Objective:To investigate the effects of massage combined with manual massage on ankle joint motion and function in patients with double ankle fractures.Methods:66 patients with double-ankle fracture treated with open surgery in our hospital from January 2020 to October 2022 were selected as the study objects,and randomly divided into a control group and an observation group by numbered envelope method according to the order of admission.The control group(n=33)received routine postoperative rehabilitation therapy.Observation group(n=33)was treated with massage therapy on this basis,and the clinical efficacy,balance function,ankle function,ankle pain degree,mobility and quality of life of the two groups were compared.Results:The excellent and good rate of the observation group was 81.82%,higher than 57.58%of the control group,and the difference was statistically significant(P<0.05).After treatment,the Baird-Jackson ankle joint score and AOFAS ankle-hind foot score in both groups were higher than before treatment,and the observation group was higher than the control group,with statistical significance(P<0.05).After treatment,the pain of Visual Analogve Scale scores in both groups were lower than before treatment,and observation group was lower than control group,the difference was statistically significant(P<0.05).After treatment,the angles of dorsalis,toe flexion,varus and valgus in both groups were greater than before treatment,and the observation group was greater than the control group,the difference was statistically significant(P<0.05).After treatment,the Berg Balance Scale score of observation group was higher than that of control group,and the difference was statistically significant(P<0.05).After treatment,the scores of physiological function,body pain,overall health and emotional function in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion:Massage combined with manual massage can effectively improve the postoperative rehabilitation effect of patients with bilateral ankle fractures,further improve ankle function and ankle joint motion,and effectively improve patients'balance ability and quality of life,which is worthy of clinical promotion.
