Comparison of the Effect of Bipolar Long Handle Artificial Femoral Head Replacement and Proximal Anti-Rotation Intramedullary Nail Implantation in the Treatment of Elderly Femoral Intertrochanteric Fractures
Objective:To investigate the effect of bipolar long handle artificial femoral head replacement and proximal anti-rotation intramedullary nail placement(PFNA)in the treatment of elderly femoral intertrochanteric fractures.Methods:A retrospective analysis was performed on 120 elderly patients with intertrochanteric fracture of femur treated in our hospital from December 2017 to December 2020.They were divided into a control group and an observation group according to different surgical methods,with 60 cases in each group.The control group underwent PFNA,and the observation group underwent bipolar long stem artificial femoral head replacement.The clinical efficacy,complications,hip function(Harris)score,visual analogue pain(VAS)score and clinical indexes of the two groups were compared.Results:The total effective rate of the observation group was 98.33%,higher than 86.67%of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 6.67%,lower than 21.67%in the control group,and the difference was statistically significant(P<0.05).1 and 3 months after operation,the Harris score and VAS score of the observation group were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).The operation time,the initial time of getting out of bed after surgery and the complete time of getting out of bed after surgery in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,with statistical significance(P<0.05).Conclusion:The use of bipolar long-shank artificial femoral head replacement for the treatment of elderly patients with intertrochanteric fractures can promote the recovery of hip joint function,and the therapeutic effect is relatively good.At the same time,it can relieve the pain of patients and reduce the incidence of complications to a certain extent,which is worthy of clinical promotion.
