多层螺旋CT诊断胸腰椎爆裂性骨折的应用价值

摘要
目的:分析多层螺旋CT诊断胸腰椎爆裂性骨折的应用价值.方法:选取本院 2019年 7月—2021年 7月检查的 95例胸腰椎爆裂性骨折患者为研究对象,采用多层螺旋CT及X线诊断,对比分析多层螺旋CT的应用效果.结果:多层螺旋CT诊断胸腰椎爆裂性骨折的检出率为 97.8%,高于X线检查的 84.2%,漏诊率及误诊率分别为 1.1%、1.1%,低于X线检查的 8.4%、7.4%,组间差异有统计学意义(P<0.05).多层螺旋CT对各类型骨折检出率均高于X线检查,组间差异有统计学意义(P<0.05).多层螺旋CT平扫轴位与多平面重建(MPR)对各类型骨折的检出率均高于容积再现(VRT),组间差异有统计学意义(P<0.05).MPR对附件骨折数、椎管分度准确数检出比例高于多层螺旋CT,组间差异有统计学意义(P<0.05).结论:胸腰椎爆裂性骨折应用多层螺旋CT诊断,可准确判断疾病,并鉴别骨折类型,为临床诊疗提供可靠证据.
Abstract
Objective:Analysis of the application value of multi-slice spiral CT in the diagnosis thoracolumbar burst fractures.Methods:95 patients with thoracolumbar burst fractures examined in our hospital from July 2019 to July 2021 were selected as the research subjects,used multi-slice spiral CT and X-ray diagnosis to compare and analyze the application effect of multi-slice spiral CT.Results:The detection rate of multi-slice spiral CT in diagnosing thoracolumbar burst fractures was 97.8%,which was higher than 84.2%in X-ray examination,the missed diagnosis rate and misdiagnosis rate were 1.1%and 1.1%,respectively,which were lower than 8.4%and 7.4%in X-ray examination,the differences were statistically significant(P<0.05).The detection rates of multi-slice spiral CT for all kinds of fractures were higher than that of X-ray examination,the differences between groups were statistically significant(P<0.05).The detection rates of various types of fractures using axial and Multi plane reconstruction(MPR)of multi-slice spiral CT were higher than those used volume rendering technique,the differences were statistically significant(P<0.05).MPR had a higher detection rate for the number of accessory fractures and the accuracy of spinal canal grading compared to multi-slice spiral CT,the differences were statistically significant(P<0.05).Conclusion:The application of multi-slice spiral CT in the diagnosis of thoracolumbar burst fractures can accurately determine the disease and distinguish the type of fracture,providing reliable evidence for clinical diagnosis and treatment.
