桃红四物汤口服联合膏药外敷保守治疗骨折的价值研究及对疼痛、炎症的影响

摘要
目的:探讨桃红四物汤口服联合膏药外敷保守治疗骨折的效果及对疼痛、炎症的影响.方法:选取 2020年 8月—2022年 8月我院收治的 118 例老年桡骨远端骨折患者为研究对象,采用随机数字表法将其分为对照组、中药组、试验组,对照组(n=40)采取常规保守治疗,中药组(n=40)在对照组基础上加桃红四物汤口服治疗,试验组(n=38)在中药组基础上加膏药外敷治疗.比较 3组的疼痛程度、血清白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平、骨折愈合时间、Cooney腕关节评分.结果:治疗 1个月后,试验组VAS评分低于中药组与对照组,中药组低于对照组,组间差异有统计学意义(P<0.05);治疗 3个月后,试验组VAS评分低于对照组,差异有统计学意义(P<0.05);治疗 6个月后,3组VAS评分比较,差异无统计学意义(P>0.05).治疗 1个月后,试验组IL-6 水平低于对照组,TNF-α水平低于中药组与对照组,组间差异有统计学意义(P<0.05).试验组骨折愈合时间短于对照组,差异有统计学意义(P<0.05).治疗 6个月后,3组Cooney腕关节评分比较,组间差异无统计学意义(P>0.05).3组优良率、治疗总有效率比较,组间差异无统计学意义(P>0.05).结论:桃红四物汤口服联合膏药外敷保守治疗可有效改善骨折早期炎症和疼痛,同时一定程度上可加速骨折愈合.
Abstract
Objective:To explore the effect of Taohong Siwu Decoction combined with plaster external application in conservative treatment of fractures and its impact on pain and inflammation.Methods:118 elderly patients with distal radius fractures admitted to our hospital from August 2020 to August 2022 were selected as the research subjects,Divide them into a control group,a traditional Chinese medicine group,and an experimental group using a random number table method.The control group(n=40)received routine conservative treatment,while the traditional Chinese medicine group(n=40)received oral treatment with Taohong Siwu Decoction in addition to the control group,the experimental group(n=38)received plaster external application in addition to the traditional Chinese medicine group for treatment.Compare the degree of pain,serum interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)among three groups Level,fracture healing time,Cooney wrist joint score.Results:1 month after treatment,the VAS score of the experimental group was lower than that of the traditional Chinese medicine group and the control group,and the traditional Chinese medicine group was lower than that of the control group,and the difference was statistically significant(P<0.05);After 3 months of treatment,the VAS score of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05);After 6 months of treatment,the VAS scores of the three groups were compared,the differences was not statistically significant(P>0.05).One month after treatment,the IL-6 level in the experimental group was lower than that in the control group,with TNF-α level was lower than that of the traditional Chinese medicine group and the control group,and the difference was statistically significant(P<0.05).The fracture healing time in the experimental group was shorter than that in the control group,and the difference was statistically significant(P<0.05);Comparison of Cooney wrist joint scores among three groups at 6 months after treatment,the differences was not statistically significant(P>0.05).Comparison of excellent rates and total effective rates among three groups,there was no statistically significant differences between groups(P>0.05).Conclusion:Oral administration of Taohong Siwu Decoction combined with plaster external application for conservative treatment can effectively improve early inflammation and pain in fractures,while also accelerating fracture healing to a certain extent.
