经跗骨窦切口钢板治疗跟骨骨折对足部功能及影像学指标的影响
Exploration of the Impact of Steel Plate Treatment Through Tarsal Sinus Incision on Foot Function and Imaging Indicators in the Treatment of Calcaneal Fractures

摘要
目的:探讨经跗骨窦切口钢板治疗跟骨骨折对足部功能及影像学指标的影响.方法:选择 2019年 8月—2023年 2 月我院诊治 56例的跟骨骨折患者为研究对象,根据随机数字表法将其分为研究组与对照组,各 28例.所有患者均给予切开复位钢板内固定术治疗,研究组采用经跗骨窦切口,对照组采用跟骨外侧L形切口.观察与记录 2组患者的足部功能及影像学指标变化情况.结果:2组手术时间对比,差异无统计学意义(P>0.05);研究组术后引流量比对照组少,术后引流时间、术后骨折愈合时间、术后住院时间均短于对照组,组间差异有统计学意义(P<0.05).术后 3个月,研究组Bohler角高于对照组,Gissane角低于对照组,组间差异有统计学意义(P<0.05).术后 3个月,研究组并发症发生率低于对照组,差异有统计学意义(P<0.05).术后 3个月,研究组足部功能优良率高于对照组,差异有统计学意义(P<0.05).结论:经跗骨窦切口钢板治疗跟骨骨折并不会增加手术难度,还可加快患者的康复进程,降低并发症的发生,能提高患者的Bohler角,降低Gissane角,可持续改善患者的足部功能.
Abstract
Objective:To explore the effects of steel plate treatment through tarsal sinus incision on foot function and imaging indicators in the treatment of calcaneal fractures.Methods:Selecting 56 patients with calcaneal fractures treated in our hospital from August 2019 to February 2023 as the research subjects,according to the random number table method,they were divided into a research group and a control group,with 28 cases in each group.All patients were treated with open reduction and steel plate internal fixation.The research group received through tarsal sinus incision,while the control group received an L-shaped incision on the lateral side of the calcaneus.Observing and recording changes in foot function and imaging indicators in two groups of patients.Results:There was no significant differences in operation time between the two groups(P>0.05);The postoperative drainage volume in the research group was lower than that in the control group,and the postoperative drainage time,fracture healing time,and hospitalization time were shorter than those in the control group,and the difference was statistically significant(P<0.05).Three months after surgery,the Bohler angle in the research group was higher than that in the control group,while the Gissane angle was lower than that in the control group,and the difference was statistically significant(P<0.05).Three months after surgery,the incidence of complications in the research group was lower than that in the control group,and the difference was statistically significant(P<0.05).After 3 months of surgery,the excellent and good rate of foot function in the research group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The treatment of calcaneal fractures with steel plates through the tarsal sinus incision does not increase surgical difficulty,but can also accelerate the patient's recovery process,reduce the occurrence of complications,improve the Bohler angle,reduce the Gissane angle,and sustainably improve the patient's foot function.
