谵妄预防护理对髋部骨折患者疼痛程度、生活自理能力及睡眠质量的影响

摘要
目的:研究谵妄预防护理对髋部骨折患者术后疼痛程度、生活自理能力及睡眠质量影响.方法:选取 2018年 6月—2020年 1月我院收治的 84例髋部骨折患者为研究对象,按照随机数字表法分为观察组和对照组,各 42例.对照组在髋部骨折术后采用常规护理,观察组在对照组基础上采用谵妄预防护理.比较 2组谵妄发生率、疼痛程度、生活自理能力及睡眠质量.结果:观察组谵妄发生率为0.00%,低于对照组的19.05%,差异有统计学意义(P<0.05).术前、术后1 d和出院前,2组患者VAS评分比较,组间差异无统计学意义(P>0.05).出院前,观察组Barthel指数及理查兹-坎贝尔睡眠量表评分均高于对照组,组间差异有统计学意义(P<0.05).结论:对老年髋部骨折患者实施谵妄预防护理,有助于降低患者髋部骨折术后谵妄发生率,提高患者的生活自理能力,改善其睡眠质量,有一定的应用价值.
Abstract
Objective:To study the effects of preventive nursing for delirium on postoperative pain level,living self-care ability and sleep quality in patients with hip fracture.Methods:84 patients with hip fracture diagnosed and treated in our hospital from June 2018 to January 2020 were selected as the study objects,and were divided into an observation group and a control group according to random number table method,with 42 cases in each group.The control group received routine nursing after hip fracture,and the observation group received delirium preventive nursing based on the control group.The incidence of delirium,pain degree,living ability and sleep quality were compared between the two groups.Results:The incidence of delirium in the observation group was 0.00%,which was lower than 19.05%in the control group,the difference was statistically significant(P<0.05).Before surgery,1 day after surgery and before discharge,there was no significant difference in VAS scores between the two groups(P>0.05).Before discharge,Barthel index and Ricchards-campbel sleep scale scores in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion:Delirium prevention nursing for elderly patients with hip fracture is helpful to reduce the incidence of delirium,improve the patients'living self-care ability and improve their sleep quality,and has certain application value.
