Serological Analysis of Brucellosis Patients with Spinal Joint Pain as the First Symptom
Objective:To explore and analyze the value of serologic detection in brucellosis patients with spinal arthralgia as the first symptom.Methods:86 cases of brucellosis in Datong Hui Tu autonomous county center for disease control and prevention from April 2020 to February 2023 were selected as the research objects,among which 36 cases of patients with spinal joint pain as the first symptom were selected as the arthralgia group,and 50 cases of patients without spinal joint pain as the first symptom were selected as the control group.Serological indexes of the two groups were detected to determine the spinal joint pain of patients and conduct correlation analysis.Results:There were no significant differences in leukocyte,erythrocyte,hemoglobin and platelet count between 2 groups(P>0.05).The contents of anti-nuclear antibody,anti-keratin antibody,anti-synoviocyclic citrulline peptide antibody and matrix metalloproteinase-3 in arthralgia group were higher than those in control group,and the contents of 1,25-dihydroxyvitamin D3 in serum were lower than those in control group,with statistical significance(P<0.05).The spinal joint pain VAS score of the arthralgia group was significantly higher than that of the control group,and the differences were statistically significant(P<0.05).Among 86 patients,Pearson analysis showed that serum levels of anti-nuclear antibody,anti-keratin antibody,anti-synovial cyclocitcitine peptide antibody,matrix metalloproteinase-3,1,25-dihydroxyvitamin D3 were correlated with VAS scores of spinal joint pain,with statistical significance(P<0.05).Binary logistic regression analysis showed that serum levels of anti-nuclear antibody,anti-keratin antibody,anti-synovial ring citcitine peptide antibody,matrix metalloproteinase-3 and 1,25-dihydroxyvitamin D3 were important factors affecting the VAS score of spinal joint pain,and the differences were statistically significant(P<0.05).Conclusion:Brucellosis patients with spinal arthralgia as the first symptom are relatively common,most of which are manifested as serum anti-nuclear antibody,anti-keratin antibody,anti-synovial cyclamate citroline peptide antibody,increased content of matrix metalloproteinase-3 and decreased content of serum 1,25-dihydroxyvitamin D3,strengthening serological detection has important clinical significance.
