肌骨超声引导精准可视化介入联合Mulligan动态关节松动术治疗肩峰下撞击综合征患者的效果分析

摘要
目的:探讨肌骨超声引导精准可视化介入联合Mulligan动态关节松动术治疗肩峰下撞击综合征患者的临床效果.方法:选择 2023年 1月—2024年 1月广东省人民医院赣州医院收治的94例肩峰下撞击综合征患者为研究对象,按照随机数字表法将患者分为对照组与试验组,每组47例.对照组采用针灸联合推拿治疗,试验组在对照组基础上采用肌骨超声引导精准可视化介入联合Mulligan动态关节松动术治疗.比较两组的疼痛程度、肩关节功能及关节活动度.结果:治疗后 7 d,两组疼痛评分均低于治疗前,且试验组低于对照组,差异均有统计学意义(P<0.05).治疗 3个月后,两组肩关节功能评分均高于治疗前,且试验组高于对照组,差异均有统计学意义(P<0.05);两组肩关节前屈、外展及外旋活动度均大于治疗前,且试验组大于对照组,差异均有统计学意义(P<0.05).结论:肌骨超声引导精准可视化介入联合Mulligan动态关节松动术能减轻肩峰下撞击综合征患者疼痛,有助于改善患者肩关节功能,扩大肩关节活动度.
Abstract
Objective:To investigate the clinical effect of musculoskeletal ultrasound-guided precise visualization intervention combined with Mulligan dynamic joint loosening in treatment of patients with subacromial impingement syndrome.Methods:A total of 94 patients with subacromial impingement syndrome admitted to Guangdong Provincial People's Hospital Ganzhou Hospital from January 2023 to January 2024 were selected as the study objects.According to random number table method,the patients were divided into a control group and an experimental group,with 47 cases in each group.The control group was treated with acupuncture combined with massage,and the experimental group was treated with musculoskeletal ultrasound-guided precision visualization intervention combined with Mulligan dynamic joint loosening.Pain degree,shoulder joint function and joint motion were compared between the two groups.Results:After 7 days of treatment,the pain scores of both groups were lower than before treatment,and the experimental group was lower than in the control group,with statistical significance(P<0.05).After 3 months of treatment,the shoulder joint function score in two groups were higher than before treatment,the experimental group were higher than the control group,the differences were statistically significant(P<0.05);shoulder flexion,abduction and external rotation of both groups were greater than before treatment,and the experimental group were greater than the control group,the differences were statistically significant(P<0.05).Conclusion:Musculoskeletal ultrasound-guided precision visualization intervention combined with Mulligan dynamic joint loosening can reduce pain in patients with subacromial impingement syndrome,help improve shoulder joint function and expand shoulder joint motion.
