摘要

目的:观察超声结合电刺激定位注射肉毒毒素治疗脑卒中后上肢痉挛患者的临床效果.方法:选取 2017年 4月—2022年 4月南京中医药大学附属张家港医院收治的60例卒中后上肢痉挛患者为研究对象,采用随机数字表法将患者分为对照组和试验组,各 30例.对照组采用电刺激定位注射肉毒毒素治疗,试验组在对照组的基础上联合超声治疗.注射后 1个月对比两组患者的上肢功能和生活质量.结果:治疗后,试验组改良Ashworth痉挛量表评分中的屈肘、屈腕、屈指评分均低于对照组,差异均有统计学意义(P<0.05).治疗后,试验组改良的生活质量量表评分中的躯体质量、心理健康、情感职能、社会职能评分均高于对照组,差异有统计学意义(P<0.05).结论:超声结合电刺激定位注射肉毒毒素治疗脑卒中后上肢痉挛,有助于改善患者的上肢活动功能,减轻卒中后的上肢痉挛,提升患者的生活质量,且没有不良反应.

Abstract

Objective:To analyze ultrasound combined with electrical stimulation injection of botulinum toxin in the treatment of upper limb spasm after stroke.Methods:A total of 60 patients with upper limb spasticity after stroke in the rehabilitation department of Zhangjiagang Hospital Affiliated to Nanjing University of Tranditional Chinese Medicine were selected.The patients were hospitalized from April 2017 to April 2022.They were divided into a control group and an experimental group with 30 cases in each group by random number method.The control group was treated with electrical stimulation and location injection of botulinum toxin,the experimental group was treated with ultrasound on the basis of the control group.After 1 month of treatment,the upper limb function and their quality of life were evaluated in both groups.Results:After treatment,the elbow flexion,wrist flexion,and finger flexion scores in the Modified Ashworth Spasticity Scale of the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the scores of physical quality,mental health,emotional function,and social function in the improved quality of life scale of the experimental group were higher than those of the control group,the differences were statistically significant(P<0.05).Conclusion:Ultrasound combined with electrical stimulation for targeted injection of botulinum toxin in the treatment of upper limb spasm after stroke can significantly improve the upper limb activity function and the quality of life of patients.
