Analysis Value of Three-Dimensional Pelvic Floor Ultrasound in Early Diagnosis of Pelvic Floor Dysfunction in Postpartum Women
Objective:To evaluate the value of pelvic floor ultrasound in early diagnosis of pelvic floor dysfunction in postpartum women. Methods:The clinical data of 120 subjects undergoing three-dimensional pelvic floor ultrasound examination in Zhangqiu District People's Hospital of Jinan city from June 2022 to May 2023 were retrospectively analyzed, and they were divided into a normal birth group,a cesarean section group and a healthy unpregnant group according to delivery conditions, with 40 cases each. All subjects underwent three-dimensional pelvic floor ultrasonography. The diagnosis rate, diagnostic efficiency and pelvic floor indexes of the three groups were compared. Results:The diagnosis rate of urinary incontinence, rectocele and uterine prolapse in vaginal delivery group were 60.00%,35.00%and 27.50%,respectively,which were higher than 37.50%,15.00%and 2.50%in the cesarean section group and 22.50%,5.00%and 2.50%in the healthy unpregnant group, and cesarean section group was higher than that in healthy non-pregnant group, the differences were statistically significant(P<0.05).Compared with the MRI,the accuracy, sensitivity and specificity of three-dimensional pelvic floor ultrasound were 92.11%,93.10%and 91.07%,respectively,and the Kappa coefficient was 0.842.The distance between the posterior wall of the bladder and the lower margin of the symphysis pubis, the angle of the posterior bladder and the distance between the ampulla of the rectum and the sphincter line of the anus in the resting state,Valsalva and anal retraction state were statistically significant among the three groups(P<0.05).The pelvic floor indexes of the vaginal delivery group were higher than those of the cesarean section group and the healthy non-pregnant group in all states, and most of the pelvic floor indexes of the cesarean section group were higher than those of the healthy non-pregnant group in all states, with statistical significance(P<0.05).Conclusion:Three-dimensional pelvic floor ultrasonography shows high accuracy, high sensitivity and high specificity in the diagnosis of pelvic floor dysfunction in postpartum women, and has certain clinical value.
