文拉法辛缓释剂与丁螺环酮治疗广泛性焦虑障碍的对照研究

摘要
目的 评价文拉法辛缓释片与丁螺环酮治疗广泛性焦虑障碍的临床疗效和安全性.方法 58例广泛性焦虑障碍患者随机分为文拉法辛缓释片组(研究组)30例、丁螺环酮组(对照组)28例,共治疗8周.采用汉密尔顿焦虑量表(HAMA)、临床总体印象量表(CGI)评定临床疗效,不良事件量表(AE)评定安全性.结果 两组治疗后第1、2、4、6、8周末HAMA量表总分与治疗前比较均降低(P＜0.01);两组的精神性焦虑因子和躯体性焦虑因子评分与治疗前评分比较均降低(P＜0.01).研究组有效率83.3％,显效率66.7％;对照组有效率85.7％,显效率67.9％,两组比较差异无统计学意义(P＞0.05).治疗后第1、2、4、6、8周末,两组CGI量表总分与治疗前比较均降低(P＜0.01),两组药物不良反应的发生率比较差异无统计学意义(P＞0.05),常见的不良反应有头痛、恶心、口干、头昏、失眠等.结论 文拉法辛缓释片与丁螺环酮片治疗广泛性焦虑障碍疗效确切,不良反应少而轻.
Abstract
Objective To compare the efficacy and safety of venlafaxine XR and buspirone in the treatment of generalized anxiety disorders (GAD).Method 58 GAD patients were randomized into study group (30 cases) treated with venlafaxine XR for 8 weeks and control group (28 cases) treated with buspirone for 8 weeks.The efficacy was assessed by using Hamilton Anxiety Scale (HAMA),Clinical Global Impression (CGI) and the safety was assessed by using Adverse Event Reporting Form (AE).Results Total scores of HAMA in both groups decreased significantly at the end of the 1 st,2nd,4th,6th,8th week of the treatment when compared with the baseline (P ＜ 0.01).Factor scores of mental anxiety and somatic anxiety in both groups decreased significantly at the endpoint when compared with the baseline (P ＜ 0.01).There were no significant differences in response rate (83.3％ vs.85.7％) and marked effective rate (66.7％ vs.67.9％) between study group and control gorup (P ＞ 0.05).Compared with the baseline,scores of CGI in both groups decreased significantly at the end of the 1st,2nd,4th,6th,8th week (P ＜ 0.01).There was no significant difference in incidence of adverse reactions between the two groups (P ＞ 0.05).The common adverse reactions were headache,nausea,dry mouth,dizziness,insomnia,et al.Conclusion Both venlafaxine XR and buspirone have good efficacy and less side effects in treatment of GAD.
