类风湿血管炎病位在脉络.营卫失调,卫不偕营,卫气壅遏,宣通不畅,湿热瘀化毒,毒损脉络是类风湿血管炎的核心病机.发病初期,以调和营卫为主,同时运用苦寒清热解毒药,荡涤毒邪,既病防传;急性期注重清热解毒通络,使营卫调和,降低疾病危重程度,防止对多系统、多脏器的损害;慢性期治宜调补营卫,益气养血通络,清解余毒,使受损脉络得以修复.调和营卫、清热解毒通络宜贯穿疾病的始终.
Analysis of Pathogenesis,Differentiation and Treatment of Rheumatoid Vasculitis from"Nutrient-Defense Disharmony,Toxin Damaging Vessels"
Rheumatoid vasculitis is located in the vessels.The core pathogenesis of rheumatoid vasculitis includes disharmony between nutrient qi and defensive qi,failure of defensive qi to accompany nutrient qi,defensive qi obstruction,failing to move smoothly,dampness-heat and blood stasis transforming into toxin,and toxin damaging vessels.At the onset of the disease,it is mainly to harmonize nutrient and defensive aspects,and at the same time,it is necessary to use bitter and cold medicinals to clear heat,remove toxin and control development of existing disease.In the acute phase,we should pay attention to clearing heat,removing toxin,dredging collaterals,harmonizing nutrient and defensive aspects,reducing the severity of the disease,and preventing damage to multiple systems and organs.In the treatment during chronic period,we should harmonize and tonify the nutrient and defense,replenish qi and nourish blood,dredge collateral and remove toxin,so that the damaged vessels could be repaired.Harmonizing the nutrient and defense,clearing heat,removing toxin and dredging collateral should run through the whole disease.
