基于三维成像技术探讨次髎、中髎深刺进针点的确定方法

目的:在前期研究的基础上,多角度综合探讨次髎、中髎深刺进针点的定位方法。方法:针刺次髎、中髎后进行CT三维重建,确保针刺入孔后,通过①同身寸或同身距离比值计算;②先确定腧穴定位再进行一定距离的位置偏移;③借助体表标志物为参考,结合相关数据进行定位;④由线到点辅助确定腧穴位置等4种方法测量并计算进针点相关的距离、角度、比值等数据,从不同的角度补充说明次髎、中髎深刺进入骶后孔的方法。结果:①根据横纵坐标骨度分寸及同身距离比值数据,次髎、中髎进针点旁开后正中线对应的同身寸分别为(1。05±0。18)寸、(0。98±0。17)寸;次髎、中髎与L5棘突下水平线的距离,与L5棘突下水平线至骶骨角距离的同身比值分别为0。29±0。03、0。50±0。08。②先确定次髎、中髎腧穴定位,其进针点再按照腧穴体表投影点纵向偏移一定距离,该偏移距离分别为(10。66±4。01)mm、(10。93±4。67)mm。③以髂后上棘为体表标志物,结合偏移角度、横向距离比值确定进针点,髂后上棘到次髎、中髎针体的连线偏离髂后上棘水平线的角度分别为(36。9± 5。52)°、(53。4±4。67)°;次髎、中髎针体到后正中线的距离与髂后上棘到后正中线距离的比值分别为0。50± 0。25、0。42±0。18。④先确定同侧八髎穴连线,再由线到点,确定次髎、中髎位置,左、右侧八髎穴连线与后矢状面的夹角分别为(7。28±4。16)°、(7。62±3。45)°。结论:通过不同的方法多角度确定次髎、中髎进针点的位置,可降低不确定性及操作失误率,降低因个体发育差异对结果的影响,更容易提高针刺入孔率,值得临床推广。
Method for Localizing Deep Needling Point of Ciliao and Zhongliao Based on Three-Dimensional Imaging Technology
Objective:To comprehensively explore the method for localizing deep needling points of Ciliao(BL 32)and Zhongliao(BL 33)from multiple perspectives based on previous studies.Methods:After Ciliao and Zhongliao were needled,three-dimensional reconstruction of CT was performed.Four methods,such as ① ratio between body cun and body distance were calculated;②the location of acupoints was determined and then a certain distance of position deviation was performed;③ using body surface markers as a reference and combining relevant data localization was performed;④four methods,such as determining the position of acupoints with line to point as assistance,were used to measure and calculate data related to the insertion point,such as distance,angle,and ratio,and methods for Ciliao and Zhongliao deep needling to enter the posterior sacral foramen were supplemented from different angles.Results:①On data and results of ratio between bone cun and body distance in horizontal and vertical coordinates:the body cun corresponding to the median line of Ciliao and Zhongliao needling points were(1.05±0.18)cun and(0.98±0.17)cun,respectively;the distance between Ciliao and Zhongliao and the horizontal line under the L5 spinous process and the distance from the horizontal line under the L5 spinous process to the sacral angle were(0.29±0.03)and(0.50±0.08),respectively.② Firstly the location of Ciliao and Zhongliao acupoints was determined,and then needling points longitudinally deviated by a certain distance according to the projection point on the body surface of the acupoints.The deviated distances were(10.66±4.01)mm and(10.93±4.67)mm,respectively.③Using the posterior superior iliac spine as a body surface marker,the insertion point was determined by combining the deviation angle and lateral distance ratio.The deviation angles of the line from the posterior superior iliac spine to the Ciliao and Zhongliao needle bodies from the horizontal line of the posterior superior iliac spine were(36.9±5.52)° and(53.4±4.67)°.The ratio of the distance from Ciliao and Zhongliao needles to the posterior midline and the distance from the posterior superior iliac spine to the posterior midline is:0.50±0.25,0.42±0.18.④First,it is to determine the line connecting the eight Liao acupoints on the same side,and then from the line to the point,to determine the positions of the second and middle Liao acupoints.The angle between the connecting line of the eight Liao acupoints on the left and right sides and the posterior sagittal plane is(7.28±4.16)°,(7.62±3.45)° respectively.Conclusions:Using different methods to determine the location of Ciliao and Zhongliao needling points from multiple perspectives,and the combined application of multiple methods,could reduce uncertainty and the error rate of operation,and reduce the influence of individual differences on the results,and improve the penetration rate of acupuncture,which should be promoted in clinic.
