目的:探讨运脾调糖方对脾虚湿瘀型2型糖尿病患者糖脂代谢的影响及临床疗效。方法:将120例脾虚湿瘀型2型糖尿病患者分为对照组和观察组各60例。对照组采用甘精胰岛素进行治疗，观察组在对照组的基础上采用运脾调糖方治疗，两组均治疗4周。统计两组治疗后中医证候疗效、糖化血红蛋白(HbA1c)达标率及治疗期间不良反应发生情况，比较两组治疗前后糖代谢指标、动态血糖波动指标、超敏C反应蛋白(hs-CRP)及脂代谢指标水平。结果:治疗后，观察组总有效率93。33％，HbA1c达标率91。67％，对照组总有效率80。00％，HbA1c达标率73。33％，两组比较，差异有统计学意义(P＜0。05)。与治疗前比较，治疗后两组血清空腹血糖(FPG)、餐后2 h血糖(2 h PG)、空腹胰岛素(FINS)、HbA1c、平均血糖波动幅度(MAGE)、平均血糖(MBG)、血糖标准差、hs-CRP、甘油三酯(TG)、总胆固醇(TC)及低密度脂蛋白胆固醇(LDL-C)水平均降低(P＜0。05)，且观察组低于对照组(P＜0。05)。治疗期间，观察组和对照组不良反应发生情况比较，差异无统计学意义(P＞0。05)。结论:运脾调糖方可有效改善脾虚湿瘀型2型糖尿病患者临床症状及糖脂代谢，降低血糖波动和机体炎症反应。
Clinical Study on Yunpi Tiaotang Prescription(运脾调糖方)on Type 2 Diabetes Mellitus of Spleen Deficiency and Dampness-Stasis Type
Objective:To study the effect of Yunpi Tiaotang Prescription(运脾调糖方)on glucose and lipid metabolism of patients with type 2 diabetes mellitus of spleen deficiency and dampness-stasis type and its clinical efficacy.Methods:A total of 120 patients with type 2 diabetes mellitus of spleen deficiency and dampness-stasis type were divided into the control group and the observation group,with 60 cases in each group.The control group was treated with insulin glargine,and the observation group was treated with Yunpi Tiaotang Prescription on the basis of the treatment of the control group.Both groups were treated for 4 weeks.The efficacy of traditional Chinese medicine syndromes,the rate of HbA1c compliance and the occurrence of adverse reactions during treatment were statistically analyzed between the two groups after treatment,and the levels of glucose metabolism indexes,dynamic blood glucose fluctuation indexes,hypersensitive C-reactive protein(hs-CRP)and lipid metabolism indexes before and after treatment were compared between the two groups.Results:After treatment,the total effective rate of the observation group was 93.33％and HbA1c compliance rate reached 91.67％,while the total effective rate of the control group was 80.00％and HbA1c compliance rate reached 73.33％,and the difference was statistically significant(P＜0.05).Compared with those indexes before treatment,serum fasting blood glucose(FPG)and 2h postprandial blood glucose(2 h PG),fasting insulin(FINS),HbA1c,mean blood glucose fluctuation range(MAGE),mean blood glucose level(MBG),blood glucose standard deviation(SD),hs-CRP,triglyceride(TG),total cholesterol(TC)and low density lipoprotein cholesterol(LDL-C)levels were all decreased after treatment(P＜0.05).The observation group was lower than the control group(P＜0.05).During treatment,there was no statistically significant difference in the incidence of total adverse reactions between the observation group and the control group(P＞0.05).Conclusion:Yunpi Tiaotang Prescription can effectively relieve the clinical symptoms and improve glucose and lipid metabolism of patients with type 2 diabetes mellitus of spleen deficiency and dampness-stasis type,and reduce the fluctuation of blood glucose and inflammation of the body.
