目的 分析氟-18-脱氧葡萄糖正电子发射计算机断层扫描(18F-FDG PET/CT)代谢参数结合血清微小RNA-22(miR-22)表达对食管癌术前TNM分期的评估价值.方法 回顾性分析2018 年2 月至2023 年2 月在本院手术治疗的 164 例食管癌患者的临床资料,所有患者术前均行18F-FDG PET/CT检查和血清miR-22 表达检测,术后病理检查均证实为食管癌并明确TNM分期.比较各期18F-FDG PET/CT代谢参数[最大标准化摄取值(SUVmax)、代谢肿瘤体积(MTV)、糖酵解总量(TLG)]和血清miR-22 表达,以病理结果为"金标准",分析18F-FDG PET/CT代谢参数结合血清miR-22 表达对食管癌术前TNM分期的评估价值.结果 T3 和T4 期食管癌患者的SUVmax大于T1 期食管癌患者(P<0.05),T4 期食管癌患者的SUVmax大于T2 期食管癌患者(P<0.05);T3和T4 期食管癌患者TLG大于T1、T2 期食管癌患者(P<0.05),T4 期食管癌患者TLG大于T3 期食管癌患者(P<0.05);N1-3 分期食管癌患者的SUVmax大于N0 期食管癌患者(P<0.05).不同T、N、M分期食管癌患者的血清miR-22 表达量比较差异有统计学意义(P<0.05),T1、T2、T3、T4 期食管癌患者血清miR-22 表达量依次减少(P<0.05).18F-FDG PET/CT代谢参数结合血清miR-22 表达评估食管癌T1、T2、T3、N、M分期的准确度均高于单项评估(P<0.05),评估食管癌T4 分期的准确度高于18F-FDG PET/CT代谢参数单独评估(P<0.05).结论 不同TNM分期食管癌患者的18F-FDG PET/CT代谢参数和血清miR-22 表达量存在差异,且 18F-FDG PET/CT代谢参数结合血清miR-22 表达对食管癌术前TNM分期的评估价值高于二者单项评估.
Objective To analyze the value of 18F-FDG PET/CT combined with serum miR-22 expres-sion in preoperative TNM staging of esophageal cancer(EC).Methods The clinical data of 164 patients with EC who underwent surgical treatment in our hospital from February 2018 to February 2023 were retro-spectively analyzed.All patients underwent preoperative 18F-FDG PET/CT examination and serum miR-22 expression detection,and the postoperative pathological examination was confirmed to be EC and the TNM staging was clarified.18F-FDG PET/CT metabolic parameters[maximum standardized uptake value(SUV-max),metabolic tumor volume(MTV)and total lesion glycolysis(TLG)]and serum miR-22 expression in each stage were compared.Using pathological results as the"gold standard",the value of 18F-FDG PET/CT metabolic parameters combined with serum miR-22 expression for preoperative TNM staging of EC was analyzed.Results The SUVmax of T3 and T4 stage EC patients was higher than that of T1 stage EC patients(P<0.05),while the SUVmax of T4 stage EC patients was higher than that of T2 stage EC patients(P<0.05).The TLG of T3 and T4 stage EC patients was higher than that of T1 and T2 stage EC patients(P<0.05),while the TLG of T4 stageEC patients was higher than that of T3 stage EC pa-tients(P<0.05).The SUVmax of N1-3 stage EC patients was greater than that of N0 stage EC patients(P<0.05).There were statistically significant differences in the expression of serum miR-22 among patients with different T,N and M stages EC(P<0.05),and the expression of miR-22 in serum of patients with T1,T2,T3 and T4 stages of EC decreased sequentially(P<0.05).The accuracy of combining 18F-FDG PET/CT metabolic parameters with serum miR-22 expression in evaluating T1,T2,T3,N and M staging of EC were higher than those of single evaluation(P<0.05),while the accuracy of evaluating T4 staging of EC was higher than that of 18F-FDG PET/CT met-abolic parameters evaluated separately(P<0.05).Conclusion There are differences in 18F-FDG PET/CT metabolic parameters and serum miR-22 expression in EC patients with different TNM stages,and the e-valuation value of 18F-FDG PET/CT metabolic parameters combined with serum miR-22 expression for pre-operative TNM staging of EC is higher than that of the two alone.
