经桡动脉冠状动脉介入治疗后穿刺点压迫管理的范围综述

目的 对经桡动脉冠状动脉介入治疗后穿刺点压迫管理相关研究进行范围综述,为控制术后出血、降低并发症发生率,减轻患者术肢不适及完善穿刺点压迫管理等提供参考.方法 检索PubMed、Cochrane Library、Web of Science、中国知网、万方和中国生物医学文献数据库中与经桡动脉冠状动脉介入治疗后穿刺点压迫管理相关的研究.检索时限为2013年2月至2023年2月.结果 最终纳入文献19篇,其中系统评价4篇、随机对照研究14篇、类实验性研究1篇.归纳出5种经桡动脉介入治疗后穿刺点压迫方法,其中最推荐气囊式压迫器,其次是螺旋式压迫器;首次放气时间建议提前至术后1 h,以最大程度提高患者舒适度、减少对术肢的影响.结论 压迫器及新型压迫方法较传统压迫方法对于经桡动脉介入治疗后穿刺点压迫更为有效,并建议将首次减压时间从术后4~6 h提前至术后1 h.未来研究可开展基于理论基础的个性化干预措施探索,以构建更加科学、合理的穿刺点压迫管理方法.
Scoping Review of Puncture Point Compression Management After Transradial Coronary Interven-tion
Objective To conduct a scoping review of studies on puncture point compression management after transradial coronary intervention,aiming to provide a reference for controlling postoperative bleeding,reducing the incidence of complications,alleviating discomforts in the operated limb and refining puncture site compression management.Methods Studies on puncture point compression management after transra-dial coronary intervention were retrieved in PubMed,Cochrane Library,Web of Science,CNKI,Wanfang Data and CBMdisc from February 2013 to February 2023.Results Finally,19 articles were included,includ-ing 4 systematic reviews,14 randomized controlled trials and 1 quasi-experimental study.Five devices for puncture point compression after transradial artery intervention were summarized.Balloon compressor was the most recommended option,followed by screw compressor.The first decompression time was recom-mended to be advanced to 1 h postoperatively to maximize patient comfort and minimize the impact on the operated limb.Conclusion The new compression method,using the recommended compressor,proves to be more effective than the traditional compression method for puncture point compression after transradial ar-tery intervention.It is advised to advance the first decompression time to 1 h from 4-6 h postoperatively.In future studies,personalized interventions may be explored grounded in theoretical principles to develop more scientific and reasonable approaches to manage puncture point compression.
