摘要
目的 构建脑卒中患者出院计划,旨为提高脑卒中患者出院护理质量提供参考.方法 2023年2-7月,在文献研究的基础上初步拟定脑卒中患者出院计划,并采用德尔菲法对来自护理管理、临床护理、健康管理等研究领域的12名专家进行2轮函询,根据专家意见修改并确定最终方案.结果 2轮专家函询的积极系数均为100%,专家权威系数分别为0.854、0.892,肯德尔和谐系数分别为0.235、0.485.经过2轮专家函询后,各条目的重要性评分为4.58～5.00,变异系数为0～0.18.最终形成的脑卒中患者出院计划方案包括5项一级条目(出院评估、出院准备、出院指导、出院随访、效果评价)、14项二级条目及44项三级条目.结论 基于文献研究及专家咨询方法构建的脑卒中患者出院计划具有较好的科学性和可靠性,可为脑卒中患者的出院全流程管理提供参考.
Abstract
Objective To construct a discharge plan for stroke patients,aiming to provide a reference for improving the post-discharge nursing quality of these patients.Methods From February to July 2023,a discharge plan for stroke patients was drafted based on literature research;2 rounds of consultation were conducted by the Delphi method with 12 experts in nursing management,clinical care and health management;the plan was revised and finalized according to expert opinions.Results For the two rounds of expert consultation,the positive coefficients were 100%,the authoritative coefficients were 0.854 and 0.892,and Kendall's coefficients of concordance were 0.235 and 0.485,respectively.After the two rounds of expert consultation,the importance of each item was scored 4.58-5.00,and the coefficient of variation was 0-0.18.The final discharge plan for stroke patients includes 5 first-level items(discharge assessment,discharge preparation,discharge guidance,discharge follow-up and effect evaluation),14 second-level items and 44 third-level items.Conclusion The discharge plan for stroke patients,developed through literature research and expert consultation,is scientific and reliable.It is a valuable reference for whole-process discharge management of stroke patients.
