目的 通过Meta分析比较肩关节镜术后应用加压冷疗与传统冷疗的疗效差异.方法 系统检索中国知网、万方、中国科技期刊数据库、PubMed、Embase、Cochrane Library、Web of Science、ClinicalTrails数据库,关于肩关节镜术后加压冷疗与传统冷疗效果比较的随机对照研究,检索时限自建库至2023年4月.由2名研究者独立进行文献筛选、质量评价和资料提取,采用Stata 15.1软件对数据进行分析.结果 共纳入7篇文献,包括442例患者.Meta分析结果显示,加压冷疗组术后24 h[SMD=-0.87,95%CI(-1.29,-0.45),P<0.001]和48 h[SMD=-1.03,95%CI(-1.53,-0.52),P<0.001]的疼痛视觉模拟评分均低于传统冷疗组;加压冷疗组术后24 h以外上髁为起点15 cm处[SMD=-0.61,95%CI(-0.92,-0.29),P<0.001]、20 cm处[SMD=-1.31,95%CI(-2.46,-0.16),P=0.026]及腋窝处[SMD=-1.19,95%CI(-2.11,-0.27),P=0.011]的肿胀值和肿胀评分[SMD=-0.86,95%CI(-1.22,-0.50),P<0.001]均低于传统冷疗组;术后48 h以外上髁为起点15 cm处[SMD=-0.81,95%CI(-1.44,-0.18),P=0.012]、20 cm处[SMD=-1.14,95%CI(-1.57,-0.71),P<0.001]及腋窝处[SMD=-1.55,95%CI(-2.95,-0.15),P=0.030]的肿胀值和肿胀评分[SMD=-0.85,95%CI(-1.44,-0.26),P=0.005]也均低于传统冷疗组.结论 对于肩关节镜术后患者,加压冷疗较传统冷疗具有更好地缓解疼痛及肿胀的作用.
Meta-analysis of Compression Cryotherapy and Conventional Cryotherapy After Shoulder Arthroscopy
Objective To compare the efficacy of compression cryotherapy and conventional cryotherapy after shoulder arthroscopy through meta-analysis.Methods Randomized controlled studies comparing the efficacy of compression cryotherapy and traditional cryotherapy after shoulder arthroscopy were systematically retrieved from CNKI,Wanfang Data,Database of Chinese Sci-Tech Periodicals,PubMed,Embase,Cochrane Library,Web of Science and ClinicalTrails from their establishment to April 2023.Literature screening,quality evaluation and data extraction were independently performed by 2 investigators.The extracted data were analyzed using Stata 15.1 software.Results A total of 7 articles were included,involving 442 patients.The Meta-analysis showed that the visual analogue scale scores 24 h[SMD=-0.87,95%CI(-1.29,-0.45),P<0.001]and 48 h[SMD=-1.03,95%CI(-1.53,-0.52),P<0.001]postoperatively in the compression cryotherapy group were lower than those in the conventional cryotherapy group;the swelling values at 15 cm from the epicondyle[SMD=-0.61,95%CI(-0.92,-0.29),P<0.001],20 cm from the epicondyle[SMD=-1.31,95%CI(-2.46,-0.16),P=0.026]and axillary[SMD=-1.19,95%CI(-2.11),-0.27),P=0.011]and swelling scores[SMD=-0.86,95%CI(-1.22,-0.50),P<0.001]24 h postoperatively in the compression cryotherapy group were lower than those in the conventional cryotherapy group;the swelling values at 15 cm from the epicondyle[SMD=-0.81,95%CI(-1.44,-0.18),P=0.012],20 cm from the epicondyle[SMD=-1.14,95%CI(-1.57,-0.71),P<0.001]and axillary[SMD=-1.55,95%CI(-2.95,-0.15),P=0.030]and swelling scores[SMD=-0.85,95%CI(-1.44,-0.26),P=0.005]48 h postopera-tively in the compression cryotherapy group were also lower than those in the conventional cryotherapy group.Conclusion Compression cryotherapy is more effective in relieving pain and swelling than conventional cryotherapy after shoulder arthroscopy.
