正畸修复联合疗法对改善深覆胎伴下前牙严重磨耗患者咀嚼功能的效果评价

摘要
目的:观察正畸修复联合治疗对深覆伴下前牙严重磨耗患者咀嚼功能的改善效果.方法:选择2018年1月-2022年1月接诊的164例深覆伴下前牙严重磨耗患者,根据治疗方案不同分为2组,即对照组72例(单纯口腔修复治疗)和试验组92例(正畸修复联合治疗).评价2组咀嚼效能,计算颞下颌关节功能障碍指数(dysfunction index,DI)、肌肉压痛指数(palpation index,PI)与颞下颌关节紊乱指数(cranio-mandibular index,CMI).评价面容美观满意度、中文版口腔健康影响程度量表(OHIP-14)评分和修复满意度评分,并比较2组不良事件发生情况.采用SPSS 23.0软件包对数据进行统计学分析.结果:治疗后,试验组咀嚼效能较对照组显著提高,DI、PI和CMI较对照组显著降低(P＜0.05).治疗后,试验组面容美观满意度和修复满意度显著高于对照组,OHIP-14评分显著低于对照组(P＜0.05).与对照组的不良事件(25.00％)相比,试验组(6.52％)降低显著(P＜0.05).结论:正畸修复联合治疗可提高深覆伴下前牙严重磨耗患者的治疗效果,改善患者咀嚼功能及颞下颌关节平衡,提高患者满意度及生活质量,减少不良事件发生风险.
Abstract
PURPOSE:To observe the effect of orthodontics combined with restoration on masticatory function in deep overbite patients with severe lower anterior teeth attrition.METHODS:From January 2018 to January 2022,a total of 164 deep overbite patients with severe lower anterior teeth attrition were collected and divided into two groups according to different treatment plans:control group(72 patients,with restoration treatment)and experimental group(92 patients,with orthodontics combined with restoration treatment).The chewing efficiency of the two groups was evaluated,temporo-mandibular joint dysfunction index(DI),muscle palpation index(PI)and cranio-mandibular index(CMI)were calculated.The satisfaction with facial esthetic,the Chinese version of Oral Health Impact Scale-14(OHIP-14)and the repair satisfac-tion score were evaluated,the occurrence of adverse events between the two groups was compared.SPSS 23.0 software package was used for statistical analysis.RESULTS:After treatment,the chewing efficiency of the experimental group was significantly improved compared to the control group,while the DI,PI,and CMI were significantly reduced compared to the control group(P＜0.05).Compared with the control group,the satisfaction degree with facial esthetic and restoration in the experimental group was significantly higher,while the OHIP-14 score was significantly lower after treatment(P＜0.05).The incidence of adverse events in the experimental group was significantly decreased compared with the control group(6.52％vs 25.00％,P＜0.05).CONCLUSIONS:Combination of orthodontics and restoration treatment can enhance the effectiveness of restoration treatment for deep overbite with severe lower anterior teeth attrition,improve the mastica-tion function and temporomandibular joint balance,satisfaction and quality of life of patients,as well as reduce the risk of adverse events.
