摘要
目的 探讨口腔常见微生物与哮喘患者免疫特征及肺功能的相关性.方法 收集2020年3月—2020年9月于上海交通大学医学院附属第九人民医院呼吸科就诊的患者共65例,将其分为哮喘组(n=45)与非哮喘组(n=20),对两组患者行口腔微生物、口腔分泌物细胞因子、肺功能检测及口腔内嗜酸性粒细胞(eosinophil,EOS)计数等相关检查,对哮喘患者的口腔常见微生物丰度差异及与其他指标相关性进行分析,揭示哪些口腔常见微生物与哮喘患者口腔内免疫特征有较强的关联性.结果 曲霉菌、乙型链球菌和流感嗜血杆菌在哮喘组口腔中的丰度明显高于非哮喘组(P<0.01),同时上述菌种与哮喘患者严重程度、哮喘控制测试(asthma control test,ACT)评分及肺功能也密切相关(P<0.05).Spearman相关性分析结果显示,哮喘患者口腔内曲霉菌、乙型链球菌和流感嗜血杆菌与口腔内EOS计数具有显著相关性(P=0.026、0.042和0.031);曲霉菌、乙型链球菌与口腔分泌液中IL-7浓度呈正相关(P=0.015、0.026);乙型链球菌和流感嗜血杆菌与口腔分泌液中IL-21浓度呈正相关(P=0.042、0.028).结论 口腔中特定的微生物与哮喘免疫特征之间存在相关性.本研究的发现将为进一步揭示这些口腔中常见菌种在塑造宿主口腔免疫环境和哮喘进展中的作用及机制奠定基础.
Abstract
Objective This study aims to explore the influence and correlation of common oral microbes on the immune characteristics and pulmonary function of asthmatic patients.Methods A total of 65 patients who visited the Respiratory Department of the Ninth People's Hospital of Shanghai in the past six months were collected.They were divided into asthma(n=45)and non-asthma groups(n=20).Oral microbial detection,oral secretion cytokine detection,pulmonary function tests,and oral EOS counts were performed on both groups.The differences in the abundance of common oral microbes in asthmatic patients and their correlation with other indicators were analyzed,aiming to reveal the strong association between common oral microbes and the oral immune characteristics of asthmatic patients.Results The abundance of Aspergillus,Streptococcus pneumoniae,and Haemophilus influenzae in the oral cavity of the asthma group was significantly higher than that in the non-asthma group(P<0.01).Moreover,these microbial species were closely related to the severity of asthma,ACT scores,and pulmonary function(P<0.05).The Spearman correlation analysis results showed a significant correlation between Aspergillus,Streptococcus pneumoniae,and Haemophilus influenzae in the oral cavity of asthmatic patients and oral EOS counts(P=0.026,0.042 and 0.031),as well as a positive correlation between Aspergillus,Strepococcus pneumoniea and the level of IL-7(P=0.015,0.026),and between Strepococcus pneumonia,Haemophilus and the level of IL-21(P=0.042,0.031).Conclusion There is a correlation between specific microbes in the oral cavity and the immune characteristics of asthma.The findings of this study will lay the foundation for further elucidating the role and mechanism of these identified oral microbes in shaping the host's oral immune pattern and the progression of asthma.
