Exploration of the clinical efficacy of Shiliuwei Kangchan Zhijing Capsules in the treatment of Parkinson's disease due to blood stasis and stirring wind with rapid eye movement sleep behav-ior disorder
Objective:To investigate the correlation between polysomnography(PSG)related parameters and the course of Parkinson's disease(PD)with rapid eye movement sleep behavior disorder(RBD)and Hoehn-Yahe stage,and to explore the clinical efficacy of Shiliuwei Kangchan Zhijing Capsules in the treatment of PD with RBD due to blood stasis and stirring wind.Methods:60 PD with RBD patients due to blood stasis and stirring wind who were treated in the En-cephalopathy Center of the First Affiliated Hospital of Anhui University of Chinese Medicine from September 2020 to April 2022 were selected.A total of 60 patients who met the inclusion criteria were divided into the treatment group and the con-trol group according to the treatment plan,with 30 cases in each group.The total sleep time(TST),sleep latency(SL),sleep efficiency(SE),wake time(WT),R-stage sleep proportion(REM%),R-stage mentalis electromyographic activity index(REAI),R-stage periodic limb movement index[RLMI(R)]and other related parameters were calculated based on PSG.The correlation between the course of PD with RBD and Hoehn-Yahe stage were analyzed.The changes of PSG param-eters,RBD scale questionnaire(RBDSQ),Pittsburgh Sleep Quality Index(PSQI)and TCM syndrome scale scores before and after treatment were evaluated in the two groups.Results:(1)Correlation analysis:TST,SE and course of disease(r =-0.684,P =0.000;r =-0.674,P =0.000),Hoehn-Yahe stage(r =-0.662,P =0.000;r =-0.628,P =0.000)showed a negative corre-lation.SL,WT,REM%,REAI PLMI(R)and course of disease(r =0.485,P =0.000;r =0.643,P =0.000;r =0.601,P =0.000;r =0.503,P =0.000;r =0.608,P =0.000),Hoehn-Yahe stage(r =0.401,P =0.110;r =0.662,P =0.000;r =0.505,P =0.000;r =0.808,P =0.000;r =0.849,P =0.000)showed a positive correlation.(2)Comparison of PSG parameters:there was no statistically significant difference in PSG related parameters between the two groups before treatment,which was compa-rable(P>0.05);SL,WT,REM%,REAI and RLMI(R)were significantly decreased,TST and SE were increased after treatment(P<0.05),and the treatment group was better than the control group(P<0.01),the difference was statistically significant.(3)Scale evaluation:there was no statistical significance in RBDSQ,PSQI scale scores and TCM syndrome scores between the two groups before treatment,which were comparable(P>0.05),and the difference was not statistically significant;after treatment,the scores of RBDSQ,PSQI and TCM syndrome scores decreased significantly(P<0.05),and the treatment group was better than the control group(P<0.01),the difference was statistically significant.Conclusion:PSG related parameters are correlated with the course of PD with RBD and Hoehn-Yahe stage.Shiliuwei Kangchan Zhijing Capsules can effectively improve the clinical symptoms of PD with RBD patients due to blood stasis and stirring wind.
