目的:评估通督调神针刺法治疗脑卒中后吞咽障碍(PSSD)的疗效和安全性.方法:计算机检索中国知网(CNKI)、万方数据库(WANFANG)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库(SinoMed)、PubMed、Co-chrane Library、Web of Science等数据库中关于通督调神针刺法治疗PSSD的随机对照临床试验(RCT),检索时限从建库至2023年10月.应用Cochrane手册中的偏倚风险评估工具评价纳入文献的质量,应用RevMan 5.4软件进行Meta分析.结果:Meta分析共纳入11篇文献,研究对象总数为1006例.研究结果显示,通督调神针刺法治疗PSSD总有效率[RR=1.26,95%CI(1.17,1.35),Z=6.51(P<0.000 01)]、洼田饮水试验评分[MD=-1.61,95%CI(-2.20,-1.02),Z=5.34(P<0.000 01)]、电视X线透视吞咽功能检查(VFSS)评分[MD=1.38,95%CI(1.09,1.66),Z=9.51(P<0.000 01)]、标准吞咽功能量表(SSA)评分[MD=-3.89,95%CI(-4.68,-3.11),Z=9.74(P<0.000 01)]均优于对照组,差异有统计学意义(P<0.01).安全性评价提示通督调神针刺法未见明显不良反应.结论:通督调神针刺法治疗PSSD的疗效可靠且相对安全,值得临床推广应用.
Meta-analysis of efficacy and safety of acupuncture for dredging governor meridian and regulating the mind for post-stroke swallow disorders
Objective:To evaluate the efficacy and safety of acupuncture for dredging governor meridian and regulating the mind for post-stroke swallow disorders(PSSD).Methods:China National Knowledge Infrastructure(CNKI),WANFANG Database(WANFANG),VIP Chinese Science and Technology Journal Database(VIP),Chinese Biomedical Literature Database(SinoMed),PubMed,Cochrane Library,Web of Scinece for randomized controlled clinical trial(RCT)on the treatment of PSSD by acupuncture for dredging governor meridian and regulating the mind were searched from the establishment of the database to October 2023.The bias risk assessment tool in Cochrane manual was used to evaluate the quality of the included literature,and RevMan 5.4 software was used for meta-analysis.Results:A total of 11 articles were included in the meta-analysis,and the total number of subjects was 1006.The results indicated that the total effective rate of acupuncture for dredging governor meridian and regulating the mind in the treatment of PSSD[RR=1.26,95%CI(1.17,1.35),Z=6.51(P<0.000 01)],the score of water swallowing test[MD=-1.61,95%CI(-2.20,-1.02),Z=5.34(P<0.000 01)],VFSS score[MD=1.38,95%CI(1.09,1.66),Z=9.51(P<0.000 01)],and SSA score[MD=-3.89,95%CI(-4.68,-3.11),Z=9.74(P<0.000 01)]were better than those in the control group,and the difference was statistically sig-nificant(P<0.01).Safety evaluation revealed no discernible adverse effects from acupuncture for dredging governor merid-ian and regulating the mind.Conclusion:Acupuncture for dredging governor meridian and regulating the mind is a depend-able and fairly safe option for treating PSSD,making it worthy of clinical application.
