Value of DWI with different parameters in preoperative evaluation of lymphovascular space invasion of endometrial carcinoma

摘要
目的:探讨弥散加权成像(diffusion-weighted imaging,DWI)不同参数术前评估子宫内膜癌淋巴脉管间隙浸润(lymphovascular space invasion,LVSI)的价值.方法:回顾并分析146例子宫内膜癌患者的临床及病理学资料,其中LVSI(-)组109例,LVSI(+)组37例.测量两组患者表观弥散系数最大值(maximum apparent diffusion coefficient,ADCmax)、ADC最小值(ADCmin)、ADC平均值(ADCmean)这3个参数,比较两组间各参数的差异,采用受试者工作特征(receiver operating characteristic,ROC)曲线比较ADCmin及ADCmean鉴别子宫内膜癌LVSI的诊断效能.结果:两组患者ADCmax差异无统计学意义(P>0.05),两组间ADCmean(F=13.832,P<0.001)及ADCmin(F=19.927,P<0.001)差异有统计学意义.ADCmin、ADCnean预测子宫内膜癌LVSI的ROC曲线的曲线下面积分别为0.853、0.772.根据最佳诊断阈值,ADCmin及ADCmean评估子宫内膜癌LVSI的灵敏度分别为86.5%及62.2%,特异度为71.6%及82.6%.ADCmin评估LVSI的诊断效能明显高于ADCmean(Z=2.221,P=0.026).结论:ADCmin及ADCmean均有助于术前评估子宫内膜癌LVSI,且ADCmin的诊断效能更高.
Abstract
Objective:To investigate the value of diffusion-weighted imaging(DWI)with different parameters in preoperative evaluation of lymphovascular space invasion(LVSI)of endometrial carcinoma(EC).Methods:The clinical and pathological data of 146 cases of EC were retrospectively analyzed,including 109 cases of LVSI(-)and 37 cases of LVSI(+).Maximum apparent diffusion coefficient(ADCmax),ADCmin and ADCmean of the two groups of patients were measured,and the differences of each parameter between the two groups were compared.Receiver operating characteristic(ROC)curve analysis was carried out to evaluate the diagnostic performance of ADCmin and ADCmean in evaluating LVSI status in EC.Results:No statistically significant difference was found in ADCmax between the two groups(P>0.05).There was significant difference in ADCmean(F=13.832,P<0.001)and ADCmin(F=19.927,P<0.001)between the two groups.The area under curve(AUC)of ADCmin,ADCmean for evaluating LVSI status in EC were 0.853 and 0.772 respectively.According to the optimal cut point,the sensitivity and specificity value of ADCmin and ADCmean were 86.5%and 62.2%,71.6%and 82.6%.The diagnostic performance of LVSI evaluated by ADCmin was significantly higher than ADCmean(Z=2.221,P=0.026).Conclusion:Both ADCmin and ADCmean are helpful for preoperative evaluation of LVSI status in EC,and ADCmin has a higher diagnostic performance.
