Clinical efficacy of three-step(blood occlusion,preventing prolapse and stopping bleeding)injection method in treatment of third-degree internal hemorrhoids
Objective:To compare the clinical efficacy of three-step(blood occlusion,preventing prolapse and stopping bleeding)injection method and internal hemorrhoids ligation in the treatment of third-degree internal hemorrhoids.Methods:A total of 72 patients with third-degree internal hemorrhoids were included and randomly divided into the experimental group and control group,36 cases in each group.The patients in the experimental group were treated with three-step((blood occlusion,preventing prolapse and stopping bleeding)injection method,and the patients in the control group were treated with internal hemorrhoids ligation.The clinical efficacy and the time to resuming normal life and work were compared between the two groups.The clinical symptoms such as hematochezia and hemorrhoids prolapse and postoperative complications including pain,pyrexia,anal swelling,and so on were evaluated.The number and the peak systolic blood flow velocity(PSV)of hemorrhoidal arteries were measured by B-mode ultrasound,and the reduction ratio of hemorrhoidal artery number and PSV were compared between the two groups.Results:During the course of the study,one patient was excluded in the experimental group,and one patient was lost to follow-up in the control group.Finally,35 cases in the experimental group and 35 cases in the control group were included in the statistical analysis.①One month after surgery,the total markedly effective rate was 100.0%in the experimental group and 97.1%in the control group,with no statistically significant difference between the two groups(P>0.05).②After surgery,the scores of hematochezia and hemorrhoids prolapse symptoms in the two groups were significantly decreased compared with those before surgery(P<0.05),and on the 7th day after surgery,the score of hematochezia symptom in the experimental group was lower than that in the control group(P<0.05).③On the 1st,3rd and 7th days after surgery,the pain score and the incidence rate of anal swelling in the experimental group were significantly lower than those in the control group(P<0.05).On the 1st and 3rd days after surgery,the defecation score in the experimental group was significantly lower than that in the control group(P<0.05).On the 1st day after surgery,the incidence rate of urinary retention in the experimental group was significantly lower than that in the control group(P<0.05).④ The time to resuming normal life and work after surgery in the experimental group was significantly shorter than that in the control group(P<0.05).⑤After surgery,the number and PSV of hemorrhoidal arteries in the two groups were significantly decreased compared with those before surgery(P<0.05),and the reduction ratios of hemorrhoidal artery number and PSV in the experimental group were significantly higher than those in the control group(P<0.05,P<0.01).Conclusion:Compared with internal hemorrhoids ligation,the three-step(blood occlusion,preventing prolapse and stopping bleeding)injection method can significantly shorten the time to resuming normal life and work after surgery in patients with third-degree internal hemorrhoids,improve the hemorrhoids prolapse and hematochezia symptoms,reduce the postoperative complications,and decrease the excessive perfusion in the hemorrhoidal artery region.
