刘铜华教授治疗自身免疫性甲状腺炎用药经验

摘要
自身免疫性甲状腺炎(Autoimmune thyroiditis,AIT)是一种自身免疫性疾病,包括桥本甲状腺炎(Hashimoto's thyroiditis,HT)、弥漫性毒性甲状腺肿(Toxic diffuse goiter)等,以桥本甲状腺炎者居多.刘铜华教授认为情志内伤及先天禀赋不足是AIT发病的关键因素,肝郁气滞火热是本病的关键病机,在临床治疗中重视疏肝理气类和清热泻火解毒类药物的使用,重视"火"类病机,重视"虚实"辨证以及心理疏导,常以自拟的甲炎康泰方为底方,辨证加减论治.本文就刘铜华教授应用甲炎康泰方化裁治疗AIT的临床经验和体会进行论述,以期指导AIT的中医治疗.
Abstract
The autoimmune thyroiditis(AIT)is an autoimmune disease,including Hashimoto's thyroiditis(HT),toxic diffuse goiter,etc.Hashimoto's thyroiditis is the majority.Professor Liu Tonghua believes that emotional internal injury and lack of innate endowment are the key factors in the onset of AIT,and liver stagnation and heat stagnation are the key pathogenesis of the disease.In clinical treatment,attention should be paid to the use of drugs for soothing liver and qi and clearing heat and fire and detoxification,the pathogenesis of"fire",the differentiation of"deficiency and deficiency"and psychological guidance,and self-designed Jiyankang Tai prescription is often used as the base prescription for syndrome differentiation and treatment.This article discusses the clinical experience and experience of Professor Liu Tonghua in treating AIT with Kangtai prescription of hypothyroidism,in order to guide the TCM treatment of AIT.
