特发性肺纤维化近年来因其治疗成本、发病率、死亡率高而备受重视,因此寻找有效且价格低廉的治疗方法和药物已经成为有待解决的难题。络病理论治疗IPF具有独特优势,基于络病理论的指导下IPF病机认识提出"络瘀"病机贯穿IPF整个病理阶段,然其物质基础尚不清楚。本文通过对特发性肺纤维化发病机制的深入研究,梳理肺毛细血管通透性增强、肺血管渗漏在IPF病理进程中的作用,并从络病理论出发,进一步揭示IPF中医"络瘀"病机的生物学基础,为络病理论指导下"通络"治法的应用奠定基础,为以络论治IPF的中医思路提供有力科学依据,为中医药治疗IPF提供新思路。
On the Biological Basis of the Pathogenesis of Collaterals and Blood Stasis in IPF Based on the Abnormality of Pulmonary Capillary Permeability
In recent years,idiopathic pulmonary fibrosis has attracted much attention because of its high treatment cost,morbidity and mortality.Therefore,finding effective and low-cost treatment methods and drugs has become a difficult problem to be solved.The theory of collaterals disease has unique advantages in treating IPF.Based on the guidance of the theory of collateral disease,it is proposed that the pathogenesis of"collateral stasis"runs through the whole pathological stage of IPF,but its material basis is still unclear.In this article,through the in-depth study of the pathogenesis of idiopathic pulmonary fibrosis,the role of enhanced pulmonary capillary permeability and pulmonary vascular leakage in the pathological process of IPF was sorted out.Based on the theory of collateral disease,the biological basis of the pathogenesis of"collateral stasis"in IPF was further revealed,lays a foundation for the application of"gredging collaterals"therapy under the guidance of collateral disease theory,provides scientific basis for TCM treatment of IPF by collateral theory,and provides new ideas for TCM treatment of IPF.
