Evaluation of the effect of bronchoscopy assisted lidocaine spraying on local anesthesia
Objective To compare the effect of local anesthesia between aerosol inhalation of lidocaine and electronic bron-choscopy assisted spraying of lidocaine in electronic bronchoscopy.Methods Totally 160 patients who were scheduled to undergo electronic bronchoscopy from March 1,2020 to March 31,2021 in the Endoscopy Center of Peking University Cancer Hospital were selected as the study subjects,and divided into the control group(80 persons)and the experimental group(80 persons).The control group received local anesthesia using conventional nebulized inhalation of lidocaine.The experimental group used electronic bronchoscopy assisted disposable endoscopic spray tube to spray lidocaine for local air-way anesthesia.The heart rate,oxygen saturation,duration of examination,times of choking,times of supplementary anesthesia spraying,patient's pain self score,patient's cough self score,operator's cough other score,and operator's oper-ation satisfaction were compared between the two groups.Results There was no significant difference in the length of ex-amination,preoperative heart rate,1-and 3-min heart rate,preoperative blood oxygen level,and the number of times of supplementary spraying of anesthesia during the operation in the two groups,all of which were P>0.05.The 1-min blood oxygen concentration in the observation group was 100.0%(99.0%-100.0%),which was higher than that in the control group,which was 98.0%(96.0%-100.0%),with Z=-5.364,and P<0.001,and the 3-min blood oxygen concentration was 99.5%(98.0%-100.0%),which was higher than that in the control group,which was 98.0%(96.0%-100.0%),withZ=-4.199,and P<0.001;the number of intraoperative choking coughs was 6.5(3.0-10.0)times,lower than that of 12.0(8.0-20.0)in the control group,Z=-4.457,P<0.001;the pain self-rating score was 2(1-3),lower than 5(3-7)in the control group,Z=-5.857,P<0.001;and the operators score for cough was 20(10-30),lower than the control group's 50(40-70)points,Z=-5.966,P<0.001.The satisfaction rate of the observation group was 80%(70%-90%),which was higher than the control group's 50%(30%-60%),with a statisti-cally significant difference,Z=-6.001,P<0.001.There were 8 patients in the control group who experienced symp-toms of hand numbness.The patient immediately stopped inhaling nebulizers and was instructed to slow down their breathing rate.The symptoms relieved and disappeared.No toxic reactions related to lidocaine were observed in all pa-tients.Conclusion The effect of electronic bronchoscopy assisted disposable endoscopic spray tube spraying lidocaine for surface anesthesia is superior to that of aerosol inhalation of lidocaine.
