Analysis of the utilization and influencing factors of traditional Chinese and Mongolian medical health services for elderly hypertensive patients in communities
Objective To investigate the use of traditional Chinese and Mongolian hypertension health management services provided by community health service institutions for the older adults with hypertension in Hohhot city and introduce the Anderson model to analyze the impact of three factors:predisposing characteristics,enabling resources,and need on the services use.The aim was to provide suggestions for improving the community health management service capabilities of traditional Chinese and Mongolian medicine and to promote the full utilization of services by older adults with hyperten-sion.Methods Using the method of cluster sampling,1 602 older adults with hypertension aged 65 and above who were registered and managed by eight community health centers in four districts of Hohhot city were selected as survey sub-jects.The Anderson Medical Service Utilization Model was used to investigate the utilization and influencing factors of traditional Chinese and Mongolian health management services in the community of older adults with hypertension.Results Finally,1 499 valid questionnaires were obtained,and the utilization rate of traditional Chinese and Mongolian medical health management services provided by the community for older adults with hypertension was 10.7%.The service items with a higher frequency of utilization were traditional Chinese medicine dietary regulation guidance(29.8%)and tradition-al Chinese medicine constitution identification(23.6%),and the overall utilization of Mongolian medicine was lower than that of traditional Chinese medicine.The results of single factor analysis showed that the utilization of health management services in traditional Chinese and Mongolian medicine was related to the awareness of hypertension treatment knowledge in traditional Chinese medicine(x2=147.645,P<0.001)and Mongolian medicine(x2=114.289,P<0.001)among eld-erly hypertensive patients,the trust of hypertension treatment of traditional Chinese medicine(x2=62.918,P<0.001)and Mongolian medicine(x2=26.111,P<0.001),the distance of reaching to the nearest community health service cen-ter(x2=13.658,P=0.001),the availability of hypertension health management services of traditional Chinese medicine(x2=572.675,P<0.001)and Mongolian medicine(x2=253.571,P<0.001)in the nearest community health service center,health self-assessment status(x2=6.240,P=0.044),self-assessment severity of hypertension(x2=48.127,P<0.001)and clinical staging of hypertension(x2=77.615,P<0.001).The results of binary logistic regression analy-sis showed that elderly people in the second stage of hypertension were 2.143 times more likely to use health management service of traditional Chinese and Mongolian medicine than those in the first stage of hypertension(P=0.045);Elderly people who knew that traditional Chinese medicine could treat hypertension were 5.445 times more likely to use health management service of traditional Chinese medicine than those who do not know(P<0.001),while Mongolian medicine was 1.760 times more likely(P=0.035);Elderly people who believed they had severe hypertension were 6.102 times more likely to use health management service of traditional Chinese medicine and Mongolian medicine than those who be-lieved they were not(P=0.003);Elderly people living in communities that provided health management service of traditional Chinese medicine and Mongolian medicine were 53.734 times more likely to use these services than those living in communities that not provided these services(P<0.001).Conclusions The traditional Chinese and Mongolian medical health management services provided by community health service institutions by older adults with hypertension in Hohhot is insufficient and uneven.The government and community health service institutions should take more effective measures to promote and expand the promo-tion and use of traditional Chinese and Mongolian medical health management services in the community of the region.
