Correlation between serum tumor markers and recurrence and metastasis in young patients with breast cancer after radical mastectomy
Objective To observe the changes of serum tumor markers after radical mastectomy for young patients breast cancer,and analyze the correlation between serum tumor markers and postoperative recurrence and metastasis.Methods Totally 129 young patients(age ≤40 years old)with breast cancer who underwent radical mastectomy in Puy-ang Third People's Hospital from January 1,2019 to August 1,2021 were selected.They were followed up for 6 months.According to the postoperative recurrence and metastasis,they were divided into recurrence and metastasis group(37 ca-ses)and non recurrence and metastasis group(92 cases).Serum related tumor markers[cyto-keratin 19 fragment antigen 21-1(CYFRA21-1),carbohydrate antigen-125(CA125),carcinoembryonic antigen(CEA),thymidine kinase 1(TK1)]were detected after surgery.The levels of serum related tumor markers were compared in patients with different recurrence and metastasis stages and TNM stages,and their correlation with TNM stage and postoperative recurrence and metastasis was analyzed.Analyzed tThe predictive value of serum CA125,CYFRA21-1,CEA,and TK1 levels for postoperative re-currence and metastasis one week after surgery was analyzed.Results Compared with the recurrence and metastasis group,the non recurrence and metastasis group had lower levels of serum CA125(t=6.506,P<0.001),CYFRA21-1(t=20.607,P<0.001),CEA(t=33.007,P<0.001),and TK1(t=18.710,P<0.001)one week after surgery.Double negative rate of estrogen receptor/progesterone receptor(ER/PR)in the recurrence and metastasis group(x2=8.543,P=0.004),human epidermal growth factor receptor 2(HER2)positivity rate(x2=5.510,P=0.019),Ki-67 positivity rate(x2=3.953,P=0.047)was were higher than the group without recurrence or metastasis.Comparison of the levels of CA125(F=8.737,P<0.001),CYFRA21-1(F=161.495,P<0.001),CEA(F=38.696,P<0.001),and TK1(F=40.642,P<0.001)were compared in patients with different TNM stages one week after surgery,with stage Ⅱ b>stage Ⅱ a>stage Ⅰ.Double negative rate of ER/PR in patients with different TNM stages(x2=14.273,P<0.001),HER2 positivity rate(x2=8.813,P=0.012),Ki-67 positivity rate(x2=10.216,P=0.006)were com-paredcomparison,with phase stage Ⅱ b>phase stage Ⅱ a>phase stage Ⅰ.Serum CA125 was positively correlated with TNM staging(r=0.653,P=0.008)and postoperative recurrence and metastasis(r=0.639,P=0.006).Serum CY-FRA21-1 was positively correlated with TNM staging(r=0.591,P=0.020)and postoperative recurrence and metastasis(r=0.627,P=0.007).Serum CEA was positively correlated with TNM staging(r=0.721,P<0.001)and postopera-tive recurrence and metastasis(r=0.684,P=0.001).Serum TK1 was positively correlated with TNM staging(r=0.622,P=0.013)and postoperative recurrence and metastasis(r=0.583,P=0.021).The combined prediction of ser-um CA125,CYFRA21-1,CEA,TK1,ER/PR,HER2,and Ki-67 had the highest area under the curve(AUC)for post-operative recurrence and metastasis,which was 0.932(95%CI:0.872-0.968,P<0.001).Conclusions The level of ser-um tumor markers in young patients with breast cancer is closely related to postoperative recurrence and metastasis.The combined detection of serum CA125,CYFRA21-1,CEA,TK1,ER/PR,HER2,Ki-67 can be used as an effective indi-cator for monitoring postoperative recurrence and metastasis.
