Research progress on predictive factors of ischemic stroke in patients with non-valvular atrial fibrillation
Atrial fibrillation(AF)is the most common arrhythmia worldwide,which can lead to various complications that threaten human health and survival.Among them,ischemic stroke(IS)is the most severe.The CHA2DS2-VASc score is a recognized standard for assessing the risk of IS in AF patients.However,with the advancement of medical research,many predictors with higher specificity have become research hotspots in recent years.This review mainly describes these predictors and explores their potential clinical application prospects.By using the CNKI and PubMed English databases,literature related to with"atrial fibrillation","cardiogenic stroke","inflammation",and"predictive indicators"from Jan-uary 1,2017,to December 31,2023 was retrieved.Inclusion criteria:(1)Risk factors for ischemic stroke in patients with AF.(2)Risk factors for left atrial appendage thrombosis in AF.(3)Pathogenesis of AF.(4)Relationship between inflammatory markers and cardiogenic stroke.Exclusion criteria:(1)Review literature from distant years.(2)Literature with poor quality and disordered syntax.A total of 50 pieces of literature were finally included.The results showed that with the progress of research on the pathophysiological mechanism of AF and the development of imaging technology,many researchers have also found many predictive indicators not included in the CHA2DS2-VASc score.Heart related markers include left atrial appendage shape and flow rate,anatomical relationship between left atrial appendage opening and surrounding tissue structure,left atrial function and structure,atrial fibrosis,left ventricular structure,left ventricu-lar dysfunction,epicardial adipose tissue,atherosclerosis,etc.Inflammatory markers such as neutrophil/lymphocyte rati-o,platelet to lymphocyte ratio,neutrophil percentage to albumin ratio,red blood cell distribution width,systemic immune inflammation index,and systemic inflammatory response index.It suggests that with the progress of research,many convenient and easy-to-measure predictive indicators have been found,which can construct a new risk scoring system or supple-ment the CHA2DS2-VASc score,but most predictive indicators need to be verified by large-scale prospective studies.
