闭环链式管理下IAPI评估量表在晚期卵巢癌患者术中的应用

摘要
目的 探讨多学科闭环链式护理管理模式下应用术中获得性压力性损伤(IAPI)风险评估量表评估晚期卵巢癌手术患者发生IAPI的应用护理效果,总结管理经验.方法 选取宁夏医科大学总医院2021 年 1 月—2022 年 1 月Ⅲ、Ⅳ期卵巢癌手术患者 40 例临床资料,采用常规术中压力性损伤管理模式及Braden评估量表作为对照组(40 例),同时收取 2022 年 2 月—2023 年 8 月的病例资料作为研究组(44 例),采用多学科闭环链式护理管理模式下IAPI评估量表,分析两组IAPI预测能力及其发生率、体位安置规范率、防护措施达标率.结果 术中压力性损伤风险评估量表评分比较,研究组中华手术护理学会(CORN)评分明显低于对照组Braden评分,IAPI发生率研究组明显低于对照组,术前术后IAPI风险评估准确率、防护措施合格率、皮肤规范交接单执行率研究组均高于对照组,差异均具有统计学意义(P<0.05).术前术后患者护理满意度研究组高于对照组,差异具有统计学意义(P<0.05).结论 多学科链式护理管理模式促进了IAPI的闭环实施,术中获得性压力性损伤风险评估量表更加准确地预测IAPI的发生,大大降低了晚期卵巢癌手术患者IAPI的发生率,使其管理更加规范化、科学化,值得临床推广应用.
Abstract
Objective To explore the application of the Intra-operative Acquired Pressure Injury(IAPI)risk evaluation scale in evaluating the nursing effect of IAPI in patients undergoing advanced ovarian cancer surgery under the multidisciplinary closed-loop chain nursing management model,and to summarize the management experience.Methods The clinical data of 40 patients with stage Ⅲ and Ⅳ ovarian cancer surgery from January 2021 to January 2022 were selected in General Hospital of Ningxia Medical University,and the conventional intraoperative pressure injury management mode and evaluation method were used in the control group(40 cases).The case data from February 2022 to August 2023 were collected as the study group(44 cases),and the intraoperative acquired pressure injury risk evaluation scale was used under the closed-loop chain nursing management model to analyze the incidence of IAPI,the standard rate of postural placement,the rate of IAPI,and the compliance rate of protective measures in the two groups.Results Compared with the scores of the intraoperative pressure injury risk evaluation scale,the CORN score in the study group was significantly lower than that in the control group,and the incidence of IAPI in the study group was significantly lower than that in the control group.The pre-and postoperative incidence of IAPI in the study group was significantly lower than that in the control group,and the accuracy of IAPI risk evaluation,the pass rate of protective measures,and the execution rate of skin standard handover orders were higher in the study group than those in the control group,and the differences were statistically significant(P<0.05).The preoperative and postoperative patient satisfaction of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusions The multidisciplinary chain nursing management model promotes the closed-loop implementation of IAPI,and the risk evaluation scale for acquired pressure injury can more accurately predict the occurrence of IAPI,greatly reduce the incidence of IAPI in patients with advanced ovarian cancer surgery,and make its management more standardized and scientific,which is worthy of clinical promotion and application.
