摘要
目的 探究对异位妊娠患者行经脐单孔腹腔镜患侧输卵管切除术后应用大承气汤治疗的临床效果.方法 回顾性分析并收集淮安市妇幼保健院妇科 2022 年 1 月 1 日—2023 年 12 月 31 日纳入符合研究标准的 75 例异位妊娠患者行经脐单孔腹腔镜下患侧输卵管切除术,其中对照组 43 例,观察组 32 例,术后予行大承气汤口服治疗,对比分析 2 组术后情况.结果 两组患者一般资料年龄、体质量指数、孕产次、既往腹部手术史(P>0.05),差异无统计学意义.观察组患者术后首次下床活动时间、术后最高体温、术后第 1 天白细胞计数和中性粒细胞比例与对照组相比(P>0.05),差异无统计学意义.观察组患者肛门首次排气时间、首次排便时间、饮食正常恢复时间、术后第 1 天视觉模拟量表(VAS)评分均显著少于对照组,术后第 1 天超敏C反应蛋白数值高于对照组(P<0.05),差异有统计学意义.两组患者术后并发症的比较(P>0.05),差异无统计学意义.结论 异位妊娠患者行单孔腹腔镜下患侧输卵管切除术后应用大承气汤治疗可以明显推进患者术后康复进程,值得在临床上推广.
Abstract
Objective To investigate the clinical effect of applying Da Cheng Qi Tang after transumbilical single-hole laparoscopic salpingo-oophorectomy of the affected side in patients with ectopic pregnancy.Methods From January1,2022 to December 31,2023 in the Department of Gynecology of Huai'an Maternal and Child Health Hospital,a total of 75 patients with ectopic pregnancy who met the study criteria were retrospectively included in the study and underwent transumbilical single-port laparoscopic salpingo-oophorectomy,of whom 43 cases were in the control group and 32 cases were in the observation group.The observation group were given Da Cheng Qi Tang oral treatment after the operation.The postoperative situation in the two groups was analyzed.Results There was no statistically significant difference in the general information of age,body mass index,pregnancy and delivery times,and history of previous abdominal surgery in the two groups(P>0.05).There were no statistically significant differences between the two groups in terms of the time of the first postoperative bed movement,the highest postoperative body temperature,the white blood cell count,and the proportion of neutrophils on the first postoperative day(P>0.05).The time to first anal exhaust,time to first defecation,time to normal diet recovery,and VAS score on the first postoperative day in the observation group were significantly less than those in the control group,the value of ultrasensitive C-reactive protein on the first postoperative day was higher than that of the control group,and the differences were statistically significant(P<0.05).The difference in postoperative complications between the two groups was not statistically significant(P>0.05).Conclusion The application of Da Cheng Qi Tang after single-hole laparoscopic salpingo-oophorectomy in patients with ectopic pregnancy can significantly promote the postoperative recovery process of the patients,which is worthy of promotion in the clinic.
