摘要
甲状腺癌属于内分泌恶性肿瘤中最常见的类型之一,而手术治疗是其最主要的治疗手段.目前,腔镜技术已被广泛采用于甲状腺癌的手术治疗中,特别是免充气腋窝入路腔镜甲状腺手术(endoscopic thyroidectomy by gasless unilateral axillary approach,ETGUA),经历十余年的发展,已证明了其出色的临床疗效和美容效果.这种手术方式可显示对甲状腺和颈侧区的良好显露,确保清晰的手术视野和简便的操作流程,现已成为甲状腺腔镜手术的主流选择之一;该术式可有效预防传统开放手术可能导致的颈部疤痕和颈部活动受限的并发症,同时可避免经口入路腔镜手术可能引起的口唇周围麻木和感觉异常.与国内广泛应用的经胸乳入路腔镜手术相比,该术式在进行单侧Ⅵ区淋巴结清扫、喉返神经和甲状旁腺的显露及保护方面更为便捷.笔者于 2022 年开展此技术,目前已行 200 余例该术式,现结合本人手术视频进行阐述.
Abstract
Thyroid cancer is one of the most common types of endocrine malignant tumors,and surgery is the main treatment method.Currently,endoscopic technology has been widely adopted in the surgical treatment of thyroid cancer.In particular,the endoscopic thyroidectomy by gasless unilateral axillary approach(ETGUA)has demonstrated excellent clinical efficacy and cosmetic outcomes after more than a decade of development.This surgical approach provides good exposure of the thyroid gland and lateral neck regions,ensuring a clear surgical field and a simple operation process,and has become one of the mainstream choices for endoscopic thyroid surgery.This technique effectively prevents complications such as neck scarring and limited neck mobility that may result from traditional open surgery,and also avoids the numbness and sensory abnormalities around the lips that may be caused by transoral endoscopic surgery.Compared with the endoscopic surgery through the chest and breast approach widely used in China,this technique is more convenient for unilateral level VI lymph node dissection and the exposure and protection of the recurrent laryngeal nerve and parathyroid glands.The authors started this technique in 2022 and have performed more than 200 cases of this procedure so far,and now explain it in conjunction with the authors'surgical videos.
