小剂量舒芬太尼复合罗哌卡因在妊娠糖尿病产妇剖宫产术麻醉中的应用

摘要
目的 观察妊娠糖尿病产妇行剖宫产术麻醉中采取小剂量舒芬太尼复合罗哌卡因的价值.方法 选取 2021 年 1 月—2022 年 1 月期间在广州医科大学附属清远医院行剖宫产术的妊娠期糖尿病产妇 80 例作为研究对象,按照随机数字表法分为对照组和观察组,每组各 40 例,对照组予罗哌卡因麻醉,观察组患者予小剂量舒芬太尼复合罗哌卡因麻醉.比较两组产妇T0(麻醉前)、T1(麻醉后 10 min)、T2(分娩后)、T3(手术结束 30 min)血糖水平,疼痛程度以及相关麻醉指标,新生儿Apgar评分及不良反应发生情况.结果 两组产妇的疼痛程度、不同时间节点的疼痛水平之间存在明显的差异;两组产妇的空腹血糖水平之间存在明显的差异,其不同时间段内的平均动脉压(MAP)、心率水平差异显著.观察组较对照组,麻醉起效时间、达到最高阻滞平面时间明显缩短,Bromage评分、麻醉维持时间明显增加(P<0.05).两组新生儿Apgar评分比较,结果显示其在不同时间段内的评分之间有统计学差异.观察组不良反应总发生率与对照组比较,无明显差异(P>0.05).结论 妊娠糖尿病产妇行剖宫产术中采取小剂量舒芬太尼复合罗哌卡因可达到更好的麻醉效果,镇痛作用明显,可有效稳定血糖指标,安全性较高.
Abstract
Objective To observe the value of low dose sufentanil combined with ropivacaine in the anesthesia of pregnant women with gestational diabetes during cesarean section.Methods 80 pregnant women with diabetes who underwent cesarean section in Qingyuan Hospital Affiliated to Guangzhou Medical University from January 2021 to January 2022 were selected as research subjects.They were divided into control group and observation group according to the random number table method,with 40 cases in each group.The control group was anesthetized with ropivacaine.The observation group was given low dose sufentanil combined with ropivacaine anesthesia.The blood glucose level,pain degree,anesthesia indexes,neonatal Apgar score and adverse reactions were compared between the two groups at times of T0(before anesthesia),T1(10 min after anesthesia),T2(after delivery)and T3(30 min after surgery).Results There were significant differences in the degree of pain and pain levels at different time points between the two groups of postpartum women.There were significant differences in the fasting blood glucose levels among parturients,and significant differences in mean arterial pressure(MAP)and heart rate levels during different time periods.Compared with the control group,the onset time of anesthesia and the time to reach the highest block plane were significantly shortened in the observation group,and the Bromage score and the maintenance time of anesthesia were significantly increased(P<0.05).In the comparison of Apgar scores between two groups of newborns,the results showed significant differences in their relevant scoring levels at different time periods.There was no significant difference in the total incidence of adverse reactions between the observation group and the control group(P>0.05).Conclusion The use of low dose sufentanil combined with ropivacaine during cesarean section in pregnant women with gestational diabetes can achieve better anesthetic effect,obvious analgesic effect,effective stability of blood sugar index and high safety.
