摘要
患者为胃窦癌,采用完全腹腔镜下远端胃切除,D2 淋巴结清扫,毕Ⅱ+布朗吻合术.手术采用倒刺线悬吊肝脏,便于术区暴露.腔镜下完成胃的切除及淋巴结清扫后,经上腹部小切口取出标本并检查切缘,同时完成空肠-空肠的布朗吻合,最后腔镜下完成胃肠吻合.对于肥胖及胸骨下角较小的患者,采用此种吻合方式非常便捷.
Abstract
The patient was diagnosed with gastric antrum cancer and underwent total laparoscopic distal gastrectomy,D2 lymph node dissection,and Billroth II+Braun anastomosis.During the surgery,barbed wire was used to suspend the liver for easier exposure of the surgical area.After completing the resection of the stomach and lymph node dissection,a small incision was made in the upper abdomen to remove the specimen and examine the surgical margins.Subsequently,under endoscopy,the Brown anastomosis of the jejunum was performed,followed by gastrointestinal anastomosis.This type of anastomosis is particularly advantageous for patients with obesity and narrow sternum angles.
