射频消融术治疗大隐静脉曲张复发影响因素分析

目的 探讨大隐静脉曲张患者采用射频消融手术治疗后出现复发的相关因素及预防措施.方法 取梅河口市中心医院 2020 年 1 月—2022 年 4 月经射频消融手术治疗后于术后 24 个月内出现复发的大隐静脉曲张患者 61 例作为复发组,按照 1:2 的配对原则进行病例对照试验设计,选取 122 例射频消融手术治疗后未复发患者作为对照组,分析影响大隐静脉曲张患者采用射频消融手术治疗后出现复发的相关因素.结果 复发组患者的病程长于对照组,复发组患者的高血脂患者占比显著高于对照组(P<0.05);复发组患者的临床表现-病因学-解剖学-病理生理学(CEAP)分级≥C3 级患者占比高于对照组(P<0.05);复发组患者的D-二聚体(D-D)值、全血黏度低切值显著高于对照组(P<0.05);Logistic回归模型结果显示:病程越长、合并高血脂、CEAP分级≥C3 级、D-D越高是术后复发的独立危险因素(P<0.05).结论 大隐静脉曲张患者采用射频消融手术治疗后出现复发的相关因素很多,尤其与病程、合并高血脂、CEAP分级≥C3级、D-D有关,应针对性采取预防措施.
Abstract
Objective To explore the related factors and preventive measures of recurrence in patients with great saphenous vein varicose veins after radiofrequency ablation surgery.Methods 61 patients with recurrent great saphenous vein varicose veins who underwent radiofrequency ablation surgery at Meihekou Central Hospital from January 2020 to April 2022 were selected as the recurrence group.A case-control trial design was conducted according to the 1:2 pairing principle,and 122 patients who did not experience recurrence after radiofrequency ablation surgery were selected as the control group.The relevant factors affecting the recurrence of great saphenous vein varicose veins in patients who underwent radiofrequency ablation surgery were analyzed.Results The duration of disease in the recurrence group was longer than that in the control group,and the proportion of patients with hyperlipidemia in the recurrence group was significantly higher than that in the control group(P<0.05).The proportion of patients with Clinical manifestations-Etiology-Anatomy-Pathophysiology(CEAP)grade≥C3 in the recurrence group was higher than that in the control group(P<0.05).The D-dimer(D-D)value and whole blood viscosity low shear value in the recurrence group were significantly higher than those in the control group(P<0.05).The results of the Logistic regression model showed that longer duration of disease,combined hyperlipidemia,CEAP grade≥C3 and higher D-D level were independent risk factors for postoperative recurrence(P<0.05).Conclusion There are many factors related to the recurrence of great saphenous vein varicose veins after radiofrequency ablation surgery,especially the course of the disease,concomitant hyperlipidemia,CEAP grade≥C 3 and D-D level.Targeted preventive measures should be taken.
