基于多分网络和网络药理学探讨中医治疗类风湿性关节炎的用药规律和作用机制

目的 系统性深入挖掘清宫痹症医案为治疗类风湿性关节炎(rheumatoid arthritis,RA)提供参考.方法 基于频数分析获得医案治疗痹症的常用药物和常见证型,在检验数据质量的基础上构建"方剂-中药"及"方剂-证型"二分网络,并构建"证-药-方"三分网络,针对挖掘结果,从中医药系统药理数据库和分析平台(Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform,TCMSP)下载核心中药的成分和靶点并通过UniProt数据库对靶点进行标准化,从GeneCards数据库获取RA靶点,利用微生信平台取交集靶点后,应用STRING数据库和Cytoscape构建蛋白互作网络(protein-protein interaction networks,PPI),并筛选核心成分和核心靶点,完成GO、KEGG富集分析.结果 共梳理清宫痹症医案347例,包括203种药物和137类证型,频次最高的10种常用药物为当归、茯苓、白芍、生甘草、生地黄、白术、黄芩、川芎、香附、陈皮,以及11种常见证型包括气血亏虚证、风寒束表证、湿盛着痹证、气机郁滞证、湿热蕴结证、肝阴不足证、血不荣筋证、湿热下注证、表寒里饮证、湿热阻络证、饮溢四肢证;多分网络挖掘出重要药物组合为当归-白芍-白术,治疗RA主要是当归中2个有效成分、白芍中13个有效成分、白术中7个有效成分,治疗RA的关键作用靶点有12个,主要涉及MAPK信号通路.结论 医案治疗RA用药方向主要为缓解疼痛、除湿、益气补血,当归-白芍-白术组合对RA具有潜在治疗效果.
Medication patterns and mechanism of traditional Chinese medicines in the treatment of rheumatoid arthritis based on mul-ticomponent network and network pharmacology
Objective To systematically analyze the medical cases of Bi syndrome in the Qing dynasty for providing a reference in the treatment of rheumatoid arthritis(RA).Methods Commonly used drugs and common syndromes of Bi syndrome were obtained based on frequency analysis.Based on the assessment of data quality,the binary networks of"prescription-traditional Chinese medicine"and"prescription-syndrome type",and a tripartitic network of"syndrome-drug-prescription"were simultaneously constructed.According to the results,the components and targets of the core Chinese medicines were downloaded from traditional Chinese medicine systems pharmacology database and analysis platform(TCMSP),and the targets were standardized through the UniProt database.RA targets were obtained from the GeneCards database,and the intersection targets were acquired through the Weishengxin platform.Protein-protein interaction networks(PPI)were constructed using STRING database and Cytoscape.Core components and core targets were screened to complete GO and KEGG enrichment analysis.Results In total,347 cases of Bi syndrome in the Qing dynasty were identified,including 203 kinds of drugs and 137 types of syndromes.The top 10 commonly used drugs in terms of frequency were Chinese Angelica,Indian Buead,White Peony Root,Liquorice Root,Rehmanniae Radix,Largehead Atractylodes Rhizomeizome,Baikal Skullcap Root,Szechwan Lovage Rhizome,Nutgrass Galingale Rhizome,and Tangerine Peel.The 11 common syndromes were Qi and Blood deficiency syndrome,Wind-Cold exterior syndrome,Dampness-Dominated Bi syndrome,Qi stagnation syndrome,Dampness-Heat accumulation syndrome,Gan Yin deficiency syndrome,syndrome of Blood failing to nourish sinews,syndrome of dampness and Heat pouring downwards,Exterior Cold and Interior Dampness syndrome,syndrome of Dampness-Heat blocking meridian and collater-als,and syndrome of fluid retention overflowing in limbs.The multi-component network showed that the crucial drug combination con-sisted of Chinese Angelica-White Peony Root-Largehead Atractylodes Rhizome.There were 2 effective components of Chinese An-gelica,13 effective components of White Peony Root and 7 effective components of Largehead Atractylodes Rhizomeizome in treatment of RA.There were 12 key targets for RA,mainly involving MAPK signaling pathway.Conclusion The main directions of medication in the treatment of RA in medical cases are pain relief,dampness-eliminating,tonifying Qi and nourishing Blood.The combination of Chinese Angelica,White Peony Root and Largehead Atractylodes Rhizome has potential therapeutic effects on RA.
