摘要
目的：探讨调强放疗联合化疗维持治疗中晚期食管癌的疗效。方法收集86例Ⅲ/Ⅳ期的中晚期食管癌患者，随机分成调强放疗联合化疗并用卡培他滨维持治疗（实验组）和调强放疗联合化疗（对照组），每组43例，2组患者均给予同一种放疗及化疗方案。在放疗第1天同时给予多西他赛75 mg/m2，第1天静脉滴注；顺铂25 mg/m2，第1～3天静脉滴注。3周为1个周期。实验组直至放疗结束后行卡培他滨维持治疗。比较2组的近期疗效及生存情况。结果实验组与对照组有效率分别为90．70％和88．37％，差异无统计学意义（ P＞0．05）。实验组1、2、3年生存率分别为83．72％、46．51％和23．26％，对照组1、2、3年生存率分别为60．47％、27．91％和11．63％，2组差异有统计学意义（χ2＝5．14，P＝0．015）。结论调强放疗联合化疗维持治疗中晚期食管癌的疗效及患者的生存情况更好。
Abstract
Objective To explore the efficacy of intensity-modulated radiotherapy combined with chemotherapy mainte-nance treatment for advanced esophageal cancer .Methods 86 stage Ⅲ/Ⅳ advanced esophageal cancer patients were collected and randomly divided into the experimental group ( intensity-modulated radiotherapy combined with chemotherapy and capecit-abine maintenance treatment ) and the control group ( intensity-modulated radiotherapy combined with chemotherapy ) ,each group had 43 cases.The patients of the 2 groups were given the same radiotherapy and chemotherapy .On the 1st day of radiotherapy ,the patients were given intravenous infusion of Docetaxelv 75 mg/m2 on day 1;and intravenous infusion of Cisplatin 25 mg/m2 on day 1-3,3 weeks was a cycle.After radiotherapy,the experimental group received capecitabine maintenance treatment .Short-term effi-cacy and survival of the 2 groups were compared.Results Effective rates of the 2 groups were 90.70%and 88.37%,the differ-ence was not statistically significant (P >0.05).The 1-,2-,3-year survival rates of the experimental group were 83.72%、46.51%and 23.26%,The 1-,2-,3-year survival rates of the control group were 60.47%、27.91%and 11.63%,there had sta-tistical difference(χ2 =5.14,P=0.015).Conclusion Intensity-modulated radiotherapy combined with chemotherapy mainte-nance treatment for advanced esophageal cancer has better efficacy and survival .
