The Influencing Factors of Postoperative Recurrence and Metastasis in Elderly Patients with Differentiated Thyroid Cancer
Objective To explore the influencing factors of postoperative recurrence and metastasis in elderly patients with differentiated thyroid cancer(DTC).Methods A retrospective analysis was conducted on the medical records of 89 elderly DTC patients who underwent surgery.All patients were followed up for 1 year.11 patients with postoperative recurrence and me-tastasis were assigned to the recurrence and metastasis group,and 78 patients without postoperative recurrence and metastasis were assigned to the non recurrence and metastasis group.Multivariate logistic regression analysis was used to analyze the influencing factors of postoperative recurrence and metastasis in elderly DTC patients.Results Single factor analysis showed that gender,surgical method,and body mass index(BMI)were not associated with postoperative recurrence and metastasis in elderly DTC pa-tients(P>0.05);Age,tumor diameter,pathological type,TNM staging,lesion type,postoperative iodine therapy,preoperative lymph node metastasis are associated with postoperative recurrence and metastasis in elderly DTC patients(P<0.05).Multivari-ate logistic regression analysis showed that age>75 years old,follicular tumor,tumor diameter≥5 cm,no postoperative iodine treatment,multiple lesions,preoperative lymph node metastasis,and TNM stage Ⅲ~Ⅵ were the influencing factors for postopera-tive recurrence and metastasis in elderly DTC patients(P<0.05).Conclusion Age>75 years old,follicular tumor,tumor diam-eter ≥5 cm,lack of postoperative iodine therapy,multiple lesions,preoperative lymph node metastasis,TNM stage Ⅲ~Ⅵ,etc.are closely related to postoperative recurrence and metastasis in elderly DTC patients.Patients with these influencing factors should be highly valued in clinical practice,and effective preventive measures should be taken to minimize postoperative recur-rence and metastasis,thereby improving patient prognosis.
