Curative Effect of Neuroendoscopic and Microscopic Resection of Transnasal Sphenoidal Approach on Pituitary Adenoma and Its Influences on PRL,GH and ACTH
Objective To compare the clinical curative effect of neuroendoscopic and microscopic resection of transnasal sphenoidal approach on pituitary adenoma(PA)and its influences on prolactin(PRL),growth hormone(GH)and adrenocortico-tropic hormone(ACTH).Methods A total of 60 patients with PA undergoing resection of transnasal sphenoidal approach were retrospectively analyzed.According to different resection methods,they were divided into microscope group(28 cases,microscopic resection)and endoscope group(32 cases,neuroendoscopic resection).The complete resection rate of tumors,perioperative inde-xes(intraoperative blood loss,operation time,hospitalization time),hormones(PRL,GH,ACTH)and postoperative complications were compared between the two groups.Results The complete resection rate of tumors in endoscope group was higher than that in microscope group(93.75%vs 71.43%,P<0.05),intraoperative blood loss,operation time and hospitalization time were all lower than those in microscope group(P<0.05).After surgery,levels of PRL,GH and ACTH in both groups were decreased,which were lower in endoscope group than microscope group(P<0.05).The incidence of postoperative complications in endo-scope group was lower than that in microscope group(9.38%vs 35.71%,P<0.05).Conclusion Compared with microscopic resection,curative effect of neuroendoscopic resection of transnasal sphenoidal approach is more significant on PA,which can im-prove perioperative indexes,reduce hormones and postoperative complications.
