3D打印辅助技术对短瘤颈复杂腹主动脉瘤的临床应用价值

目的 探究3D打印辅助技术对短瘤颈复杂腹主动脉瘤(AAA)的临床应用价值.方法 回顾性分析短瘤颈复杂AAA患者60 例,根据是否采用3D辅助打印技术将上述研究对象划分为常规干预组(n =32)、3D打印组(n = 28);常规干预组患者接受腹主动脉CT血管造影(CTA)检查;3D打印干预组在常规干预组的基础之上联合 3D打印技术辅助检查.比较两组术前预测、术中测量得到的瘤颈最大直径、瘤腔最大直径、瘤体最大直径、瘤颈与正常腹主动脉夹角(a角)、瘤颈与腹主动脉中轴夹角(b角)之间的差异;记录并比较术前、术后 3 个月患者真腔最大直径、假腔最大直径、瘤体最大直径.结果 两组研究对象瘤颈最大直径、瘤腔最大直径、瘤体最大直径的术前评估结果与术中测量结果比较差异均无统计学意义(P>0.05);然而常规干预组a角、b角术前评估与术中测量结果有一定差异性(P<0.05);两组患者术后3 个月与术前相比,真腔直径扩大、假腔直径以及瘤体直径缩小,且3D打印组变化更加显著(P<0.05).结论 3D打印技术辅助腹主动脉CTA可精准还原重建图像,对短瘤颈AAA患者术前评估、手术方案的抉择以及手术效果具有积极的影响,值得临床借鉴应用.
Clinical Application Value of 3D Printing Assisted Technology for Short Neck Complex Abdominal Aortic Aneurysm
Objective To explore the clinical application value of 3D printing assisted technology for short neck complex abdominal aortic aneurysm(AAA).Methods A total of 60 patients with AAA of short proximal neck were retrospectively ana-lyzed.According to using 3D printing assisted technology or not,they were divided into routine intervention group(n =32)and 3D printing group(n = 28).The routine intervention group underwent abdominal aortic CT angiography(CTA),while 3D printing group was additionally given 3D printing assisted technology.The differences in the results between preoperative prediction and in-traoperative measurements[the maximum diameters of tumor neck,tumor cavity and tumor,the angle between tumor neck and normal abdominal aorta(angle a),the angle between tumor neck and the middle axis of abdominal aorta(angle b)]were com-pared between the 2 groups.The maximum diameters of true cavity,false cavity and tumor were recorded before and at 3 months after surgery.Results There was no significant difference between the preoperative evaluation results and the intraoperative measurement results of the maximum diameter of tumor neck,cavity and body between the 2 groups(P>0.05).However,there were some differences between preoperative evaluation and intraoperative measurement of Angle a and Angle b in the conventional intervention group(P<0.05).Three months after surgery,the true cavity diameter expanded,the false cavity diameter and the tumor diameter shrank in both groups,and the changes were more significant in the 3D printing group(P<0.05).Conclusion The application of 3D printing assisted technology in abdominal aortic CTA can accurately restore and and reconstruct images,which play positive roles on preoperative evaluation,choice of surgical plans and surgical effect in patients with AAA of short proximal neck.
