Medium and Long-term Efficacy of Laparoscopic Hepatectomy in Patients with Small Hepatocellular Carcinoma and Its Effect on T Cell Lymphocyte Subsets
Objective To explore the medium and long term efficacy of laparoscopic hepatectomy in patients with small hepatocellular carcinoma and its effect on T cell lymphocyte subsets. Methods A total of 112 patients with small hepatocellular carcinoma were randomly divided into the study group and the control group. The control group received open hepatectomy, the study group was treated with laparoscopic hepatectomy.Compared the indicators of surgery[amount of bleeding, time of operation, hospital stays],the ORR and DCR,the quality of life, the survival rates of the 3th year and the 5th year after operation, the liver function[alanine aminotransferase(ALT),total bilirubin(TBIL),aspartate aminotransferase(AST),direct Bilirubin(DBIL)],the T cell lymphocyte subsets[CD3+cell,CD4+cell,CD8+cell].The postoperative complications of the 2 groups were observed and counted. Results After treatment, the ORR,DCR,the quality of life, the survival rates of the 3th year and the 5th year, the level of CD3+cell,the level of CD4+cell were higher, the amount of bleeding, the time of operation, the hospital stays, the level of ALT、the level of AST、the level of TBIL、the level of DBIL and the level of CD8+cell of the study group were lower than those of the control group(P<0.05).The incidence of postoperative complications of study group were lower than control group(P<0.05).Conclusion The laparoscopic hepatectomy in the treatment of small hepatocellular carcinoma can effectively reduce the amount of bleeding and hospital stays, increase the quality of life and long-term survival rate of patients. At the same time,it can also effectively improve the liver function and immune function.
