Analysis of Related Influencing Factors of Esophageal Stenosis After Endoscopic Submucosal Dissection for Early Esophageal Cancer
Objective To analyze the relevant influencing factors of esophageal stenosis after endoscopic submucosal dissection(ESD)for early esophageal cancer.Methods 98 patients with early esophageal cancer were selected,all of whom un-derwent ESD treatment.The incidence of esophageal stenosis was recorded after surgery,and a multivariate logistic regression a-nalysis model was used to analyze the influencing factors of esophageal stenosis after ESD surgery in early esophageal cancer.Re-sults Among 98 early esophageal cancer patients treated with ESD,16 cases developed postoperative esophageal stenosis,with an incidence rate of 16.33%(16/98);Among them,10 cases(62.50%)developed esophageal stenosis within 30 days after surger-y,4 cases(25.00%)developed esophageal stenosis within 30~60 days after surgery,and 2 cases(12.50%)developed esopha-geal stenosis beyond 60 days after surgery;There were 16 cases of esophageal stenosis,including 5 cases of mild stenosis(31.25%),8 cases of moderate stenosis(50.00%),and 3 cases of severe stenosis(18.75%).Univariate analysis showed that age,gender,focus location,body mass index(BMI),diabetes,hypertension,pathological type,and delayed bleeding were not relat-ed to the occurrence of esophageal stenosis after ESD in patients with early esophageal cancer(P>0.05);The longitudinal diame-ter of the lesion,the depth of tissue infiltration,and the postoperative wound surface are related to the occurrence of esophageal stenosis after ESD(P<0.05).Multivariate logistic regression analysis showed that the main influencing factors for the occurrence of esophageal stenosis in early esophageal cancer patients after ESD were the longitudinal diameter of the lesion≥50 mm,the depth of tissue infiltration m3 sml,and the postoperative wound surface>3/4 of the esophageal circumference(P<0.05).Con-clusion The incidence of esophageal stenosis in early esophageal cancer patients after ESD treatment is relatively high,and there are many influencing factors,such as longitudinal length of the lesion ≥ 50 mm,tissue infiltration depth of m3 sml,and postopera-tive wound surface>3/4 of the esophageal circumference.It is necessary to attach great importance to the above factors in clinical practice and take proactive preventive measures to ensure the physical and mental health of patients as much as possible.
