Predictive Value of Expression of Serum TLR4 and ICAM-1 on Postoperative Pulmonary Infection in Patients with Bone Tumors Under General Anesthesia
Objective To analyze the predictive value of the expression of serum regulation of TOLL-like receptor4(TLR4)and intercellular cell adhesion molecule-1(ICAM-1)on postoperative lung infection in patients with bone tumors.Meth-ods 40 patients who developed pulmonary infection after bone tumor surgery were selected as the observation group,and the se-verity of infection was classified according to the clinical pulmonary infection score(CPIS),and the observation group was divided into mild infection group(n=23)and severe infection group(n=17),and 40 cases of healthy patients received physical exami-nation at the same period were selected as the control group.To compare the differences in serum TLR4 and ICAM-1 levels among the 3 groups and analyze the value of serum TLR4 and ICAM-1 levels in predicting postoperative pulmonary infection in patients with bone tumors.Results The levels of TLR4 and ICAM-1 in the control group,mild infection group,and severe infection group gradually increased(P<0.05).Pearson test showed that serum TLR4 and ICAM-1 levels were positively correlated with pulmona-ry infection in patients with bone tumors after general anesthesia(γ=0.812,0.814,P<0.05).The results of multifactorial logis-tic regression analysis showed that serum TLR4 and ICAM-1 levels were independent risk factors for the development of pulmonary infections in patients with bone tumors after surgery(OR=4.963,4.938,P<0.05).The results of ROC curve analysis showed that the area under the curve of serum TLR4 and ICAM-1 levels predicting postoperative pulmonary infection in patients with bone tumors was 0.806 and 0.840 with 95%CI of 0.703-0.886 and 0.741-0.912,which had good predictive efficacy.Conclusion The expression of serum TLR4 and ICAM-1 is significantly increased in patients with postoperative pulmonary infection in bone tumor patients,which is an independent risk factor for the development of postoperative pulmonary infection in bone tumor pa-tients,and their high levels predict postoperative pulmonary infection in bone tumor patients.
