Clinical Evaluation Value of Thrombelastogram Combined with Serum MMP-9 and VEGF for Coagulation Function in Elderly Patients with Advanced Lung Cancer
Objective To explore the evaluation value of thrombelastogram(TEG)combined with serum matrix metallo-proteinase(M MP)-9 and vascular endothelial growth factor(VEGF)for coagulation function in elderly patients with advanced lung cancer.Methods A total of 91 elderly patients with advanced lung cancer were enrolled as lung cancer group.According to presence or absence of hypercoagulability,they were divided into hypercoagulability group(n=58)and non-hypercoagulability group(n=33).A total of 91 healthy controls during the same period were enrolled as control group.The four blood coagulation in-dexes[prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB)],TEG relat-ed indexes[coagulation reaction time(R),formation time of blood clot(K),coagulation angle(a angle),the maximum amplitude(MA),clotting index(CI)],serum MMP-9 and VEGF in different groups were detected and compared.The relationship between TEG related indexes,serum MMP-9,VEGF and 4 blood coagulation indexes was analyzed by Pearson correlation analysis The pre-dictive value of TEG related indexes,serum MMP-9,VEGF and combined detection for hypercoagulability in elderly patients with advanced lung cancer was analyzed by receiver operating characteristic(ROC)curves.Results PT and APTT were the shortest in hypercoagulability group,followed by non-hypercoagulability group and control group,while levels of FIB,MMP-9 and VEGF were on the contrary(P<0.05).There was no significant difference in TT among the three groups(P>0.05).R and K were the lowest in hypercoagulability group,followed by non-hypercoagulability group and control group,while a angle,MA and CI were on the contrary(P<0.05).Pearson correlation analysis showed that MMP-9,VEGF,a angle,MA and CI were negatively correlated with PT and APTT,while positively correlated with FIB(P<0.05).R and K were positively correlated with PT and APTT,while negatively correlated with FIB(P<0.05).The results of ROC curves analysis showed that area under the curve(AUC)values of serum MMP-9,VEGF,R,K,a angle,MA and CI for predicting hypercoagulability were 0.747,0.717,0.753,0.723,0.777,0.739 and 0.726,respectively.AUC and sensitivity of combined detection were 0.902 and 96.55%,higher than those of single index(P<0.05).Conclusion TEG related indexes,serum MMP-9 and VEGF are all related to coagulation function in elderly patients with advanced lung cancer,and combined detection of the above indexes can improve clinical evaluation value for hyper-coagulability.
