Risk Factors of Postoperative Bladder Spasm in Patients with Non Muscle Invasive Bladder Cancer
Objective To explore the risk factors of postoperative bladder spasm in patients with non muscle invasive bladder cancer,and provide basis for clinical prevention and treatment of bladder spasm.Methods The clinical data of 96 pa-tients with non muscle invasive bladder cancer who underwent transurethral resection of bladder tumor were retrospectively ana-lyzed.They were divided into spasticity group and non spasticity group according to whether bladder spasm occurred.The data such as gender,age,tumor stage,number of tumors,duration of indwelling catheter,etc.of the 2 groups of patients were collected,and the single factor analysis was performed first.Then the variables with statistical differences were included in the multivariate Logistic regression model,and finally the risk factors of bladder spasm in patients with non muscle invasive bladder cancer after surgery were obtained.Results Three days after operation,30 of 96 patients with non muscle invasive bladder cancer developed bladder spasm,accounting for 31.25%(30/96).Among them,there were 16 mild spasms,8 moderate spasms,and 6 severe spasms.The patients with multiple tumors,preoperative urinary tract infection,catheter retention time>7 days,bladder Detrusor muscle instability,and preoperative anxiety in the spasticity group were higher than those in the non spasticity group,with a statis-tical significant difference(P<0.05).Multivariate Logistic regression analysis showed that multiple tumors,preoperative urinary tract infection,indwelling catheter time>7d,detrusor instability,preoperative anxiety were risk factors for postoperative bladder spasm in patients with non muscle invasive bladder cancer(OR>1,P<0.05).Conclusion Multiple tumors,preoperative urina-ry tract infection,catheter retention time>7 days,detrusor muscle instability,and preoperative anxiety are risk factors for postop-erative bladder spasm in patients with non muscle invasive bladder cancer.Clinical attention should be paid to these factors and targeted intervention should be carried out to prevent or reduce the occurrence of bladder spasm.
