Expression of Serum TgAb and TPOAb in Differentiated Thyroid Carcinoma Patients and Their Relationship with Central Lymph Node Metastasis
Objective To investigate the expression of serum TgAb and TPOAb in differentiated thyroid carcinoma pa-tients and its relationship with central lymph node metastasis.Methods A total of 100 patients with differentiated thyroid cancer were selected as the observation group,and 50 healthy volunteers were selected as the control group at the same time.The serum TgAb and TPOAb levels were compared between the 2 groups.The influencing factors of central lymph node metastasis were ana-lyzed.ROC curve was used to analyze the value of serum TgAb and TPOAb levels in predicting central lymph node metastasis of differentiated thyroid cancer.Results The serum levels of TgAb and TPOAb in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).There were 56 patients with central lymph node me-tastasis and 44 patients without central lymph node metastasis.There were significant differences between the metastasis group and the non-metastasis group in tumor size,TgAb,TPOAb,whether the tumor was multifocal,and whether the tumor was bilateral(P<0.05).There were no significant differences in age,gender,nerve invasion,vascular invasion,preoperative TSH level,and histo-logical classification between the 2 groups(P>0.05).The results of multivariate analysis showed that multifocality,bilaterality,high expression of TGAb,high expression of TPOAb,and low preoperative TSH were independent risk factors for central lymph node metastasis(P<0.05).The AUC of serum TgAb,TPOAb levels and their combination in predicting central lymph node me-tastasis of differentiated thyroid cancer were 0.771,0.752 and 0.819,respectively.The sensitivity and specificity of combined prediction were 84.9%and 89.8%,which were higher than those of single prediction(P<0.05).Conclusion Serum TgAband TPOAb levels are significantly up-regulated in differentiated thyroid carcinoma patients,and their high expression is an independ-ent risk factor for central lymph node metastasis.
